Abstract The pathophysiological process in Alzheimer’s disease and other amyloid disorders usually involves the transformation of a soluble monomeric protein via potentially toxic oligomers into amyloid fibrils. The structure and properties of the intermediary oligomers have been difficult to study due to their instability and dynamic equilibrium with smaller and larger species. This has hampered the development of a deeper understanding of the molecular pathophysiology of the disorders and strategies for their treatment. In hereditary cystatin C amyloid angiopathy (HCCAA), a cystatin C variant is deposited in arterial walls and causes brain hemorrhage in young adults. Although carriers of the cystatin C variant may be easily identified by genetic testing, no treatments are available. In the present investigation, redox experiments involving a recombinant double-cysteine mutant of cystatin C were used to generate stable oligomers (dimers, trimers, tetramers, decamers and high-molecular-weight oligomers) and these were characterised concerning size, shape and function. The results showed the oligomers to be highly ordered, domain-swapped assemblies of cystatin C and that the oligomers could not build larger oligomers, or fibrils, without domain swapping. The stabilised oligomers were used to induce antibody formation in rabbits. Oligomer-specific antibodies were obtained and these could be used to selectively remove cystatin C dimers from biological fluids containing both dimers and monomers. A miniaturised high-throughput system was developed and used to test 12 monoclonal antibodies and 1040 drugs in a clinical drug library for their capacity to reduce cystatin C dimer formation in vitro. Several candidates for treatment of HCCAA could be identified. In a different set of patients, scheduled for elective surgery, the plasma levels of cystatin C, creatinine and four inflammatory markers were studied in order to investigate the influence of inflammation on cystatin C, to evaluate its usefulness as a marker of glomerular filtration rate (GFR, ‘kidney function’). The cystatin C level did not change significantly during the observation period (seven days) and did not correlate with the level of any of the four inflammatory markers, and thus, the inflammatory status of a patient does not influence the role of cystatin C as a marker of GFR. Amyloidosis is a systemic disorder characterized by extra cellular deposition of a protein-derived material, known as amyloid, in multiple organs. Amyloidosis occurs when native or mutant poly- peptides misfold and aggregate as fibrils. The amyloid deposits cause local damage to the cells around which they are deposited leading to a variety of clinical symptoms. There are at least 23 different proteins associated with the amyloidoses. The most well-known type of amyloidosis is associated with a hematological disorder, in which amyloid fibrils are derived from monoclonal immunoglobulin light-chains (AL amyloidosis). This is associated with a clonal plasma cell disorder, closely related to and not uncommonly co-existing with multiple myeloma. Chronic inflammatory conditions such as rheumatoid arthritis or chronic infections such as bronchiectasis are associated with chronically elevated levels of the inflammatory protein, serum amyloid A, which may misfold and cause AA amyloidosis. The hereditary forms of amyloidosis are autosomal dominant diseases characterized by deposition of variant proteins, in dis- tinctive tissues. The most common hereditary form is transthyretin amyloidosis (ATTR) caused by the misfolding of protein monomers derived from the tetrameric protein transthyretin (TTR). Mutations in the gene for TTR frequently re- sult in instability of TTR and subsequent fibril formation. Closely related is wild-type TTR in which the native TTR protein, particu- larly in the elderly, can destabilize and re-aggregate causing non- familial cases of TTR amyloidosis.

Liu6,7 Abstract Background: Cystatin C is a well-validated marker of glomerular filtration rate in chronic kidney disease. Higher plasma concentrations of cystatin C are associated with worse clinical outcomes in heterogenous populations of critically ill patients and may be superior to creatinine in identifying kidney injury in critically ill patients. We hypothesized that elevated levels of plasma cystatin C in patients with acute respiratory distress syndrome (ARDS) would be associated with mortality risk. Methods: In a retrospective study, cystatin C was measured by nephelometry on plasma obtained at enrollment from 919 patients in the Fluid and Catheter Treatment Trial. Multivariable logistic regression was performed testing the association between quartiles of cystatin C and 60-day mortality. Analyses were stratified by acute kidney injury (AKI) status identified in the first 7 days after enrollment by Kidney Disease: Improving Global Outcomes (KDIGO) criteria. Results: Cystatin C was significantly higher among those patients who died compared to those who survived to 60 days [1.2 (0.9–1.9) mg/L vs. 0.8 (0.6–1.2) mg/L, p < 0.001]. Compared to the lower three quartiles, subjects in the highest quartile of cystatin C had a significantly higher odds of death at 60 days [OR 1.8 (1.2–2.6), p = 0.003 in adjusted analyses]; the odds of death incrementally rose in higher cystatin C quartiles compared to the lowest quartile (OR 1.1, 1.8, and 2.5).

Irrespective of the clinical presentation, afected patients manage a chronic and life-threatening condition that severely impacts their quality of life. Although the primary symptoms and diagnostic criteria for ATTR are increasingly being discussed in the medical literature, due in large part by continual advances in uncovering disease pathophysi- ology, there exists a surprising paucity of published data on the patient journey and family experience. In order to address this disparity, two focus groups, one for ATTR-CM and one for ATTR-PN, were convened and asked to describe the diagnostic process, symptoms, and impact on their own quality of life that was experienced from these rare and typically misdiagnosed illnesses. Results: Patients in both ATTR groups often underwent a long and difcult diagnostic odyssey characterized by seemingly nonspecifc physical manifestations resulting in mismanagement and suboptimal care, inadequate interventions, and delays in establishing the correct diagnosis, which was integral to determining the specialized treatment they needed. Collectively, patients with ATTR-CM and patients with ATTR-PN reported a similar number of symptoms, but the type of symptoms varied.

The aim of this study was to determine the predictive value of cystatin C for the presence and severity of CAD and the association between this protein and other biochemical risk factors for atherosclerosis in patients with suspected CAD. Methods: Ninety-four patients with CAD, and 92 patients without CAD but with cardiovascu- lar risk factors, were included in this study. Echocardiography and other pertinent laboratory examinations were performed. Subjects were divided into four groups according to their cystatin C quartile. Cystatin C groups were analyzed for the association with CAD characteristics. Results: The number of patients with CAD increased as the quartile of cystatin C increased, and there was a remarkable difference between quartiles (p < 0.001). Logistic regression analy- sis revealed independent predictors of incident CAD as cystatin C, hs-CRP, eGFR, HDL choles- terol and SBP (p = 0.005, p = 0.027, p = 0.017, p = 0.014 and p = 0.001, respectively). Moreover, cystatin C concentration was significantly correlated with CAD severity score (b = 0.258, p < 0.01). A cut-off value of 0.82 mg/L for cystatin C predicted incident CAD with a sensitivity and specificity of 75.5% and 75.0% respectively.

To the best of our knowledge, this is the first work that synthesizes and critically evaluates existing evidence on the possible association between human gut microbiota and cognitive function in the elderly. For this purpose, comprehensive literature searches were conducted using the electronic databases PubMed, Google Scholar, and ScienceDirect. Beel, MD, PhD1 The development of new immunomodulatory therapies and their implementation in the treatment of mul- tiple myeloma in the past years, offer new perspectives for the treatment of other plasma cell dyscrasias. Light chain amyloidosis is historically associated with a very poor prognosis, despite the small size of the monoclonal plasma cell population, due to progressive amyloid deposition in vital organs. Hence, advances in treatment are eagerly awaited. Luckily, myeloma patients are paving the way for light chain amyloidosis treatment, clearly demonstrating that immunomodulatory drugs and proteasome inhibitors are capable of controlling plasma cell proliferation. Two recently published trials have shown a remarkable survival benefit with CyBorD, a bortezomib containing regimen in light chain amyloidosis, possibly setting a new standard for the treatment of this disease. In this article, we review current insights in the pathogenesis, diagnostic challenges, prognostic markers and available treatments for light chain amyloidosis.
