SUBCONTRACTOR PREQUALIFICATION DOCUMENTS The following items in this package provide direction to complete the subcontractor prequalification process with BIG Construction. • BIG - Insurance Requirements • BIG - Construction Safety Manual • Sample W-9 Form • Sample OSHA’s Form 300 1 SUBCONTRACTOR AND SUPPLIER INSURANCE REQUIREMENTS Adequate Certificates of Insurance, with additional insured endorsements attached, in carriers having an A.M. Best rating of A-/VII or better and licensed to do business in the State of Illinois, shall be filed with General Contractor before commencement of Subcontractor’s Work. Name of Certificate Holder/Project Description TBD Name of Certificate Holder: To Be Determined Project Description: To Be Determined COMMERCIAL GENERAL LIABILITY: General Aggregate (per project) $2,000,000; Products & Completed Operations Aggregate 2,000,000; Personal/ Advertising injury $1,000,000; Each Occurrence $1,000,000 *The policy must include all required ADDITIONAL INSUREDS on a primary/non-contributary basis employing Additional Insured Endorsement (CG 2010 and CG 2037) OR equivalent(s) covering ongoing and completed operations. *A Waiver of Subrogation in favor of BIG Construction, LLC and others as required by contract BUSINESS AUTO LIABILITY: $1,000,000 combined single limits OR $1,000,000/$1,000,000 Bodily Injury each person/accident and $1,000,000 Property Damage each accident *Additional insureds on a primary basis UMBRELLA/EXCESS LIABILITY: $5,000,000 Each Occurrence $5,000,000 General Aggregate If Subcontractor work includes the scopes of any demolition, excavation, earth retention, cast-in-place concrete (only if contract value is $5,000,000), precast concrete, masonry, steel, roofing, curtainwall, electrical, plumbing, fire protection, HVAC, Elevator or spray foam insulation the following limits are required: $10,000,000 each Occurrence $10,000,000 General Aggregate WORKER’S COMPENSATION: In accordance with the laws of the state of Illinois including Employer’s Liability insurance with a limit not less than the following: $1,000,000 Bodily injury by Accident (each accident); $1,000,000 Bodily Injury by Disease (policy limit); $1,000,000 Bodily Injury by Disease (each employee) *A Waiver of subrogation in favor of BIG Construction, LLC and others as required by contractor BIG Construction, LLC | 1021 West Adams Street, LL3 | Chicago, Illinois 60607 EMPLOYER’S LIABILITY $1,000,000 each accident; $1,000,000 each employee-disease; $1,000,000 each policy limit-disease PROFESSIONAL LIABILITY/ERRORS AND OMISSIONS: Professional Liability/Errors and Omissions – Subcontractor, if Subcontractor's own employees render the design or engineering professional services included in the Work, or Subcontractor's design consultants, if Subcontractor's own employees do not render such services, shall maintain, at all times while services contemplated by this Agreement are being completed and for ten (10) years after Substantial Completion, a professional liability/errors and omissions liability insurance policy for professional services rendered. Whether such insurance is written on a claims-made basis or a project specific basis, the retroactive date shall be prior to the start of Subcontractor's Work. Renewal policies during this period shall maintain the same retroactive date. Such insurance shall provide not less than $2,000,000 coverage per claim and $2,000,000 coverage per year in the aggregate POLLUTION LIABILITY Pollution Liability covering third-party injury and property damage claims, including cleanup costs and mold exposures, as a result of pollution conditions arising from the operations and completed operations of the Subcontractor and its sub-subcontractors. Completed operations coverage for pollution liability must remain in effect for at least ten years after final completion of the Subcontract Work. The pollution liability insurance must have a limit of not be less than $2,000,000 per occurrence and $2,000,000 in the aggregate. The Subcontractor must include the General Contractor and Owner as additional insureds under the pollution liability policy. ADDITIONAL INSURED Commercial General Liability and Excess Umbrella Liability policies described above shall include the following as additional insured, including their officers, directors, and employees (the spelling of these parties must be exactly correct): BIG Construction, LLC (General Contractor) Owner - TBD Architect - TBD Others ‘to be determined’ (if needed to complete your quote, please contact BIG Construction bidding representative to confirm the exact count) TOOL& EQUIPMENT Only if required. Tool and Equipment Insurance for whatever fire or extended coverage or other property insurance Subcontractor deems necessary for protection against loss of owned or rented capital equipment, facilities (which term does not include the Subcontract Work), and tools, including any tools owned by mechanics and any tools, equipment, scaffolds, bracings, stagings, towers, forms, and similar items owned or rented by the Subcontractor. The Subcontractor must require its sub-subcontractors to maintain similar insurance on similar items owned or rented by its sub-subcontractors. General Contractor will not have any liability with respect to such equipment, facilities, and tools. The requirement to secure and maintain this tool and equipment insurance is solely for the benefit of the Subcontractor and its sub-subcontractors. Failure of the Subcontractor or its sub-subcontractors to secure this insurance or to maintain adequate levels of coverage does not obligate the Contractor for any losses of owned or rented equipment. *If building requirements exceed BIG’s limits, Subcontract must comply per project insurance requirements. Visitor Policy Bloodborne Pathogens Policy PURPOSE In response to the hazards associated with workplace exposure to pathogens from blood and other bodily fluids, BIG Construction has set these guidelines to protect our workers in the course of first aid treatment, healthcare and occupied space construction, etc. It is BIG Construction’s policy to treat any and all possible exposures to blood and bodily fluids as a serious threat to health. RESPONSIBILITIES PROJECT TEAM • Provide all BIG Construction employees with bloodborne pathogen training and information via healthcare and/or site orientation, pre-construction meetings, etc. • Coordinate with applicable staff to ensure that existing areas under construction have been thoroughly cleaned (e.g. Certificates of Clean). • Report any bloodborne related incidents to BIG Construction’s Insurance Department as soon as reasonably possible. The Project Team should also complete the required incident reporting documentation and forward this information to the Insurance Department. • Coordinate post-exposure procedures with the affected parties, relevant staff, and/or BIG Construction Safety De p a rt m e nt . • Direct any blood-borne-pathogen-related questions or concerns to the BIG Construction Safety Department. GENERAL REQUIREMENTS DEFINITIONS • Blood – human blood, blood components, and any products made from human blood. • Bloodborne Pathogens – pathogenic microorganisms that are present in human blood and can cause disease in humans. These include but are not limited to the hepatitis B virus (HBV) and human immunodeficiency virus ( HIV). • Contaminated – the presence or reasonably anticipated presence of blood or other potentially infectious materials on an item or surface. • Contaminated Laundry – laundry which has been soiled with blood or other potentially infectious materials, or may contain sharps. • Contaminated Sharps – any potentially contaminated object which could penetrate the skin, including but not limited to needles, scalpels, broken glass, broken capillary tubes, and exposed dental wires. • Decontamination – the use of physical or chemical means to remove, inactivate, or destroy bloodborne pathogens on a surface or item to the point where they are no longer capable of transmitting infectious particles, and the item is rendered safe for handling, use, or disposal. • Engineering Controls – controls (e.g. self-sheathing needles, sharps disposal containers, etc.) that isolate or remove the bloodborne pathogens hazard from the workplace.
