Social inequalities are a considerable strain on any society and economy. Therefore, different forms of social security systems have been applied by nations, religious institutions, or private companies for centuries in order to mitigate the many negative effects of poverty. Internationally, it is now recognized by most countries that some form of social security mechanisms are crucial to a country’s healthy and effective social and economic development. On this background, the Konrad-Adenauer-Stiftung (KAS) is promoting the concept of Social Market Economy also internationally as a consideration and inspiration for policy makers and representatives of the private sector, who influence the economic and social development of different countries around the world. Sharing experiences in economic policy drafting and balancing state-led social security schemes with the preservations of individual and market freedoms is therefore at the core of KAS’ international efforts to strengthen the global link between economic growth and social justice. This publication addresses the situation in Uganda and provides a comprehensive presentation of the strength and weaknesses of the Ugandan social security systems. It gives an insight into the development and situation of both formal and informal security systems in Uganda and analyses the different available options as well as the remaining gaps. We hope, this publication will make a significant contribution to the complex and thriving discourse on the development of a Ugandan social security system, and wish for the deep and enriching impact the authors have envisioned. Farming and pastoralism will continue to play a predominant role in Karamoja’s livelihoods portfolio for the foreseeable future. Agriculture is not a livelihoods sector that will change quickly or dramatically. 2. Though mining and tourism may improve state revenues and personal income, the two sectors are not robust enough to generate many new jobs or livelihood opportunities in and of themselves. 3. A transition from pastoralism to settled agro-pastoral livelihoods has been taking place in the last decade and will continue. However, the transition is painful in terms of cultural and traditional resistance to changing ways of life and, in particular, gender roles. 4. A strategic focus on behaviour change is therefore integral to easing livelihood transitions as market linkages grow and the influence of the state increases. NOTE ON METHODOLOGY AND OBJECTIVES This report utilizes strategic foresight techniques to build three scenarios for the future of livelihoods in Karamoja. Through desk research and key informant interviews with government officials and representatives from civil society, a number of key drivers were identified that impacted the quality and quantity of livelihood growth in Karamoja. These drivers were ranked on a matrix charting their potential impact versus the amount of uncertainty they may face over the next five years. Hypotheses were constructed for each key driver and used to build narratively cohesive scenarios. The objective for this report is to inform strategic responses to each scenario in an attempt to identify one response that can flexibly adapt to an indeterminate future.

The aim of the study was to determine the period prevalence, pattern and perceptions of cleft lip and cleft palate in children born between 2005 and 2010 in two hospitals in Kisoro District, Uganda. Methods: The study involved a retrospective review of medical records of mothers who delivered live babies between January 2005 and December 2010 in Kisoro Hospital and St. Francis Hospital, Mutolere in Kisoro District. Key informant interviews of mothers (n = 20) of the children with cleft lip and/or clip palate and selected medical staff (n = 24) of the two hospitals were carried out. The data were analysed using descriptive statistics. Results: Over the 6 year period, 25,985 mothers delivered live babies in Kisoro Hospital (n = 13,199) and St. Francis Hospital, Mutolere (n = 12,786) with 20 babies having oro-facial clefts. The overall period prevalence of the clefts was 0.77/1,000 live births. Sixty percent (n = 12) of children had combined cleft lip and palate and the same proportion had clefts on the left side of the face. More boys were affected than girls: 13 versus 7.

Whereas HMIS quality concerns like the accuracy, completeness and timeliness of reports have been more commonly assessed and reported about in a number of studies, relatively less documentation is found on the actual utilisation of the information generated from HMIS reports. Yet, the HMIS is not an end in itself but just a tool to inform managers and enable them take informed and timely decisions. This study assessed the utilisation of HMIS data for decision making at the grassroots level in Bufumbira East Health Sub-District (HSD) of Kisoro District. It was found that HMIS data were not used for decision making at the point of collection and that the HMIS was dogged by many problems like few dedicated staff. The staff lacked sensitization on the HMIS and were not trained in completing the reports and data analysis. Lower level units submitted their data directly to the district bypassing the HSD. The HMIS was not planned for and lacked funding and stationery. HMIS functioning was not a subject for support supervision and there was only verbal feedback from the district level.

Quality care saves lives and resources of both the patient and the health system. However, many developing countries have neglected diagnostics, especially clinical imaging. Non-use of or poor imaging services lead to wrong diagnosis and treatment, unnecessary health expenditures, and poor health outcomes. Regional Referral Hospitals should have good diagnostic imaging services to provide expert care and bridge the continuum of care by stemming the flow of uncomplicated cases from general hospitals to National Referral Hospitals. Aim: To determine the situation of diagnostic imaging services at regional referral hospitals in Uganda. Methods: A descriptive cross-sectional survey of all the nine Regional Referral Hospitals of Uganda was done in 2007. Environmental inspection of the imaging units, process observation and exit interviews with 156 patients present on the day of the visit were done to assess their satisfaction with the imaging services. Staff were also interviewed. However, quality validation of the imaging outputs was not done. Selected health facilities in the capital Kampala were visited to obtain data on patients referred from upcountry. Five-year hospital records were reviewed for outputs.
