Annual Re-registration Phi Lambda Sigma Beta Delta Chapter will submit its annual re-registration application during the re-registration period following the election of new officers.

Purpose The purpose of Phi Lambda Sigma is to support pharmacy leadership commitment by recognizing leaders and fostering leadership development.

Activities ● New Student Orientation: Each year, the School of Pharmacy hosts orientation for incoming P1 students. The PLS President and President-Elect, the PLS mentoring co-chairs, and the president collaborated with the Assistant Dean of Students, Dr. Sharon Corey, and the staff of The Curran Pharmacy Student Center to integrate the Student Mentoring Program into orientation activities. ● First Year Pharmacy Student Mentoring Program: This program serves as a welcome to the incoming first year students to ease the transition as they begin their pharmacy education. Students are matched with P1 mentees and asked to initiate an email conversation three weeks before orientation to introduce themselves and give a few pieces of initial advice. Over the next several weeks, mentors and mentees communicate over email and social media, and then met in person during orientation. ● Fall and Spring Leadership Retreats: The Leadership Retreats are hosted by PLS in conjunction with APhA-ASP. The retreat objectives included promoting leadership and collaboration to increase student involvement in organizational events related to the Dean’s Theme. ● James P. and Nancy A. Mastrian Young Professionals Lecture: PLS and the Alpha Omicron Chapter of Rho Chi collaborate to host an annual lecture relating to healthcare and aimed towards young professional students. The lecture is designed to broaden the student body’s understanding of important topics in healthcare as well as help student pharmacists become more engaged in current public health issues and develop their leadership skills. ● Professional Development Workshop Series: PLS members design and host a series of workshops which feature guest faculty members and other distinguished members in the profession of pharmacy. These workshops work to improve student skills within both professionalism and within leadership. ● Chapter Meetings: Chapter meetings are held on Tuesday roughly once a month during the academic year. ● Charity Auction: This event has become a well known social tradition within the School of Pharmacy, in addition to being the largest student-run philanthropic event. One hundred percent of the proceeds from the event are donated to the Grace Lamsam Foundation, which provides pharmacy services to the underserved population of Pittsburgh via multiple free clinics. Students and faculty have the opportunity to bid on various items and events donated by faculty. ● Pirates Cookout Faculty Events: The Beta Delta Chapter holds an annual Pirates Tailgate for faculty. The event is donated by the chapter and bid on by Pitt School of Pharmacy faculty members and their families in support of the Grace Lamsam charity auction that takes place in February. ● Service: PLS participates in occasional service events, namely making dinners at the the Ronald McDonald house by the UPMC Children’s Hospital in Pittsburgh.

Membership Requirements 1. Membership is open to currently enrolled Pitt students as defined herein. 2. All members must accept and comply with all of the requirements and limitations of registration as a condition of membership. 3. Membership in Phi Lambda Sigma is voluntary and all risk of personal injury, property damage or other losses that occur incidental to membership or participation in activities are assumed solely by the student organization and/or individual member(s) or non-member participant(s). Accordingly, the , its officers and agents shall not be responsible nor liable for any claims or causes of action for damage or loss of property or personal injury of any kind or nature which may arise out of or are incidental to the conduct of any organization's activities or that of any individual's participation in any group-related activity. it is further understood and agreed that it is the responsibility of the officers of the organization to assure that all of the organization's members and non-member participants in all activities sponsored by Phi Lambda Sigma are fully informed and advised of this ASSUMPTION OF RISK, and in the event any individual member or participant should express or indicate non-acceptance, the organization's officers shall forbid participation and/or membership of such individual. In regard to the organization member or non-member participant Assumption of Risk, Phi Lambda Sigma acknowledges that the University recommends that the organization members and non-member participants make every effort to arrange for the acquisition of liability insurance sufficient to protect all participants against those risks being assumed. For sport-related organizations, the University recommends that each individual have insurance sufficient to allow for any risks by participation, accident or by deficiencies in physical health. 4. No hazing or illegal discriminatory criteria for membership, including those listed in the University of Pittsburgh's non-discrimination policy, will be used as a condition for membership in the organization. These factors include race, color, religion (except for religious organizations which may restrict membership to those who share their faith-based beliefs), national origin, ancestry, sex (except same sex organizations permitted under Title IX), age, marital status, familial status, sexual orientation, gender identity and expression, genetic information, disability or status as a veteran. This non-discriminatory policy will be applied in a manner consistent with the requirements of the First Amendment of the United States Constitution and other laws applicable to student organizations. 5. Eligibility for membership requires that applicant have finished at least one year of professional school and has maintained a 2.5 grade point average on a 4.0 scale. 6. There are national membership dues required.

Membership Procedures Students are first introduced to Phi Lambda Sigma during the Back to School Picnic, which takes place the first week of classes. In October/November, an introductory presentation is given to anyone interested in applying. Applications open Mid-November/Early-December and are due the first Friday in January. Applications are posted and all information is submitted on the Pitt Pharmacy Porxtal student website. Students will be informed of their membership by the first week in March. An induction ceremony will take place at the end of March, which will include an oath and swearing in of new officers. Qualifications for student applicants include a GPA of at least 2.5, 1 semester of pharmacy coursework completed, and an ability to demonstrable leadership. Components of the application include submission of a CV or résumé, sponsorship by a current member of PLS, 2 short essays, a photo, and an optional letter of recommendation by a faculty member.

● The election of new members shall occur annually, preferably in the spring of each year. ● A quorum for the election of new members shall be two-thirds (2/3) of the voting membership as defined by the local by-laws. A three-fourths (3/4) affirmative vote of the membership present shall be required for approval of new members. ● Nominations for membership shall come from any voting member of the local chapter. ● Individuals shall become members of the Society upon complying with membership requirements, approval of membership by the active members of local chapters, payment of initiation fees, and being duly initiated. ● A membership certificate and a key pin shall be given to each member after being duly initiated. ● The chapter secretary shall, upon election of new members, forward to the National Office membership request forms provided for that purpose by the National Office.

Voting Privileges Voting privileges are limited to currently enrolled Pitt student members in good standing with the organization. Voting privileges apply to any current active member of PLS. Voting privileges are lost if a member is removed or resigns from PLS.

Associate Membership ● Faculty Membership: Those members of the staff, faculty and administration of schools and colleges of pharmacy who shall be initiated by local chapters or were formerly initiated and presently serve as staff, faculty or administrators in schools and colleges of pharmacy. ● Alumni Membership: Those members of the Society who were initiated during their collegiate enrollment. ● Honorary Membership: Those individuals who have demonstrated dedication, leadership, and service to the profession of pharmacy, including those members pursuing degrees in pharmaceutical sciences. ● Associate members may not vote, hold office nor serve in a formal leadership role. They may participate in organization meetings and activities. Pitt student membership shall comprise of at least 75% of the total membership at all times.

Terms of Office Terms of PLS office will begin at the Induction Ceremony, taking place at the end of March to the induction ceremony of the following year, where the transition and swearing in of new officers occurs.

Officer Responsibilities and Duties President: Responsibilities include submitting documents and announcement forms to the National Organization, requesting meeting times and rooms, serving as liaison to the national chapter, planning the Induction Ceremony in coordination with Rho Chi, representing local chapter at the National Delegates meeting at the APhA annual meeting, and overseeing activities of other officers and committee chairs.

Vice-President: Responsibilities include assisting the President with managerial activities, running the Professional Development Workshop Series, and recruiting speakers for the PLS/Rho Chi Professional Lecture Series.

President-Elect: This position will replace the president during the following academic year. Responsibilities of this position include shadowing and assisting the President, being included in official correspondence, and completing the PLS Leadership Challenge.

Secretary: Responsibilities include recording meeting minutes, taking roll at PLS events, and submitting articles to the Pitt Capsule and national newsletter.

Treasurer: Responsibilities include balancing the official PLS budget and submitting checks and payments to the School of Pharmacy account.

Auction Chair and Co-chair: Responsibilities include planning and running the PLS Annual Auction, which takes place in February.

Mentor Chair and Co-chair: Responsibilities include running PharmD New Student Orientation in conglomeration with the School of Pharmacy and assisting faculty with the PharmD Candidate Interviews.

Election of Officers ● Officers are elected in March before new members and officers are sworn in at the Induction Ceremony. The positions that are available are President-Elect, Vice-President, Treasurer, Secretary, Mentor Co-chair and Auction Co-chair. Elect and Co-chair positions are only available to first year PharmD students because they will transition the following year. ● Students applying for a position will formally announce their intent to run to the president a week prior to the March PLS members meeting. ● Elections will take place at the March PLS members meeting. Voting will be done by the PLS members, with each member counting as one vote. Candidates for each position with the most number of votes will be awarded the office. ● President, Mentor Chair, and Auction Chairs will assume their offices as elects and co-chairs from the previous year. There will be no formal vote for these offices.

Voting Powers of Officers All officers have the power to vote on bylaw changes.

Removal of Officers

Membership in the society may be revoked at any time by the inducting chapter or the Executive Committee for proven behavior that is in conflict with the goals and purposes of Phi Lambda Sigma. Just cause must be established and due process must be followed. The following steps are recommended to ensure the member’s rights and privacy and protected: 1. Confidential investigation by a committee--appointed preferably by the chapter, but as necessary, by the Executive Committee. 2. Report of committee exonerating or preferring specific charges. 3. Formal notification to the accused member. 4. Formal hearing. 5. Recommendation of the committee to the inducting chapter, or as applicable, the Executive Committee. 6. Notification of the outcome to the accused member by the inducting chapter or Executive Committee.

Revocation of membership shall be determined at a meeting of the inducting chapter by at least 2/3 votes for revocation, or by 2/3 votes of the Executive Committee.

Additionally, the Executive Committee shall be provided a summary of the revocation documenting the revoked member, describing just cause and due process, the number of members present, the number of active members in the chapter, the votes for revoca

Vacancies If a vacancy occurs in an officer position, the void will be filled by an individual nomination and subsequent chapter vote, with the winner receiving the greatest number of votes. A ⅔ quorum is required for voting. The newly elected officer will continue to fulfill their duties until the end of the normal term, being the transition at the Induction ceremony.

Committees There are no committees to report.

Meetings Chapter Meetings: Chapter meetings are held on the second Tuesday of each month during the academic year

Finances Phi Lambda Sigma shall fully comply with the University fundraising policies and procedures for registered student organizations. Every year, Phi Lambda Sigma hosts an Auction for School of Pharmacy faculty, residents, and students. Students and faculty submit prizes to be auctioned off at the event. Students can bid for faculty and resident items, and faculty can bid for student-submitted items. All proceeds of the auction go to the Grace Lamsam Pharmacy Program. All fundraising and budgetary spending will reflect the views and mission of the Pittsburgh School of Pharmacy and will be used for the benefit of the faculty and students. The organization will not maintain an outside bank account if it receives funding from the Graduate and Professional Student Government or the University of Pittsburgh School of Pharmacy.

Publications Code ​ In compliance with the Publications Code for Student Organizations, all publications of the organization will comply with current copyright laws, be distributed according to policies and procedures, refrain from expressions that are considered obscene or libelous according to current statute, advocate or insight the material interference or physical disruption of the educational process of the peace, order and decorum of the campus, or that advocate or incite imminent, lawless action or the violent overthrow of the government and identify the organization as publisher and specify that the group is a registered student organization at the University of Pittsburgh

Advisor Randall B. Smith, PhD, Senior Associate Dean The advisor is responsible for offering guidance and oversight to the organization’s activity. They will be informed of all budgetary spending and events. The advisor is chosen by the Dean of the School of Pharmacy and will be removed from office voluntarily or by a unanimous vote from the acting members of PLS, where each member counts as one vote.

External Affiliations The organization is affiliated with the University of Pittsburgh School of Pharmacy.

Bylaws Membership Procedures ● Applications are separated into folders based on their graduation year ● Current PLS members will review the applications, comparing individuals within the same graduation year ● All current members will have a formal meeting where they will discuss each individual applicant and compare them against other applicants in their graduation class ● When a new candidate is introduced, the PLS member who serves as that applicant’s sponsor will speak on their behalf, talking about why that candidate would be a good or bad candidate for PLS ● Afterwards, a timed open discussion will take place, weighing the applicant’s strengths and weaknesses as a leader ● A ballot will be passed out where members will award 1, 3, or 5 points to each of the applicants, with 5 points being the best score ● Awarding 5 points to an applicant means that they are a strong candidate for PLS, 1 point means that they should most likely not qualify for a good candidate in PLS, and a 3 is a neutral opinion. ● Scores are tallied and are shared anonymously ● At the discretion of the PLS members, there will be a cutoff for how many members will enter for their respective classes

Amendments Proposed amendments shall be presented to the National Office in writing no less than sixty (60) days prior to the annual meeting of the House of Delegates. The National Office shall distribute the proposed amendments to all chapters no less than thirty (30) days prior to the annual meeting of the House of Delegates at which the proposed amendments shall be submitted for vote. Section 2. Amendments to the Constitution shall require an affirmative vote of two-thirds (2/3) of the delegates and voting Executive Committee members present at the annual meeting of the House of Delegates. All constitution additions, revisions and deletions must be reported to the SORC.
