• First you must be an Adaptive Sports USA Chapter Member • Sanctioning applications will be submitted via the Adaptive Sports USA Website. • Information needed when submitting a request • Chapter info • Event info/dates/budget estimate • Sports/level requesting • Lead officials info for each sport • Insurance Fees

• Chapter Membership fee: $225 • based on a calendar year (Jan-Dec) • Sanctioning fee: • Chapter Membership. • $150 per event • Additional late fees will apply to requests made less than 30 days prior to the start date.

• Insurance • No event only insurance will be offered this year. Chapter insurance is available at an additional charge. Please contact Shelli at [email protected] if interested in Chapter insurance for the 2020 season. • Proof of insurance will be required for sanctioning 1 2 3 4 5
