Neuroendocrine tumors of the larynx represent a rare group of neoplasms characterized by pathological and biological heterogeneity. The histological and immunohistochemical diagnosis is the most important step in the appropri- ate management of these tumors and the prognosis varies according to histological types. Conventional anatomical and func- tional imaging can be complementary for diagnosis, staging and monitoring of treatment response. MATERIAL AND METHODS. Here we report on a case of a laryngeal neuroendocrine small cell carcinoma occurring in a 67-year-old man who was referred to our clinic for clinical reevaluation, diagnosis and treatment. The clinical presentation, the histopathological and immunohistochemical examination and management of this kind of tumor are highlighted. CONCLUSION. Small cell neuroendocrine carcinomas are very aggressive neoplasms. Patients could benefit from surgery, but radiotherapy and chemotherapy remain the treatment of choice. Very low incidence of neuroendocrine tumors in the larynx and specifically very poor prognosis of neuroendocrine small cell carcinoma encouraged an extensive literature review.     24 Neuroendocrine Neoplasms Natasha Rekhtman, MD, PhD* General Definitions The subject of neuroendocrine neoplasms, starting with the definition of what neuroendocrine means, is thoroughly confusing to the beginner. This chapter reviews the basic concepts and definitions pertaining to this subject. Let us start with a definition of neuroendocrine. As the term implies, there are two components: “neuro” and “endocrine.” The “endocrine” quality refers to the secretory nature of neuroendocrine cells: they produce and secrete peptides and amines. The “neuro” quality refers to their ultrastruc- tural similarity to neurons: neuroendocrine cells store their secretory products in granules (i.e., dense-core granules), which bear resemblance to synaptic vesicles. Neuroendocrine cells are dif- ferent from neurons structurally (no processes) and by the fact that the secretory mode is paracrine rather than synaptic. Also note that not all that secretes is neuroendocrine: for example, thyroid and adrenal cortex are not neuroendocrine because their cells do not possess neurosecretory granules (they are simply endocrine). Thus, at the most basic level, neuroendocrine cells are defined as the presence of neurosecretory granules in nonneurons. Tumors derived from these cells have a char- acteristic “neuroendocrine morphology” and share expression of “neuroendocrine markers.” Neuroendocrine Markers In the past, neurosecretory granules were identified by electron microscopy and special stains. Currently these methods have been completely supplanted by immunohistochemical markers. These are called neuroendocrine markers and they include synaptophysin (SYN), chromogranin (CHR), neural-specific enolase (NSE), and CD56 (SYN and CHR specifically recognize dense-core granules). Note that these markers also recognize true neurons and neu- roblastic cells (primitive neurons).

This disease is misdiagnosed as malignant lymphoma in up to one-third of cases and is as- sociated with the development of systemic lupus erythematosus (SLE). Methods: The medical literature between the years 1972 and 2014 was searched for KFD, and the data were collected and analyzed regarding the epidemiology, clinical presentations, diagnosis, management, and suggested diagnostic and treatment algorithms. Results: Although KFD has been reported in other ethnic groups and geographical areas, it is more frequently diagnosed in young women of Asian descent. Patients with the disease typically present with rapidly evolving tender cervical lymphadenopathy, night sweats, fevers, and headache. Diagnosis is based on histopathological examination. Excisional lymph node biopsy is essential for a correct diagnosis. Apoptotic coagulation necrosis with karyorrhectic debris and the proliferation of histiocytes, plasmacytoid dendritic cells, and CD8+ T cells in the absence of neutrophils are characteristic cytomorphology features. Interface dermatitis at the onset of KFD may be a marker for the subsequent evolution of SLE. The natural course of the disease is typically benign. There have been a handful of studies reporting on the utility of a variety of immunohistochemical markers that distinguish between the two entities. Achaete-scute complex-like 1 (MASH1, ASCL1) is important in the development of the brain and the diffuse neuroendocrine system including pulmonary neuroendocrine cells. A recent study, using a cDNA array, identified Mash1 as one of the best classifier genes to differentiate pulmonary small cell carcinoma from Merkel cell carcinoma. We immunohistochemically applied this finding to the diagnostic setting. A total of 30 cases of Merkel cell carcinoma and 59 cases of small cell carcinoma of the lung were immunostained with anti-MASH1 and TTF-1 antibodies. Of 59 small cell carcinomas, 49 (83%) expressed MASH1 in nuclear staining whereas out of 59 small cell carcinomas, 43 (73%) expressed TTF-1 in nuclear staining. MASH1 was completely negative in all 30 Merkel cell carcinomas whereas TTF-1 expression was seen in 1 of the 30 Merkel cell carcinomas (3%). MASH1 is a useful adjunct marker for differentiating small cell carcinoma of the lung from Merkel cell carcinoma.
