Gymnasium Repair & Renovation Services at Various Baltimore City Public Schools Proposal Due Date: Thursday, November 6, 2014 at 11:00 A.M. Local Time. In order to be eligible for consideration, bids must be received at City Schools Office of Materials Management no later than 11:00 am local time, November 6, 2014, in Room 401. Vendors mailing bids shall allow sufficient carrier delivery time to ensure timely receipt of their bid in the Office of Materials Management (Room 401) prior to the deadline. Any bid received in the Office of Materials Management after the submission deadline, no matter what the reason, will be returned unopened. Delivery to the City Schools’ mailroom, lobby etc shall not constitute delivery to the Office of Materials Management located in Room 401. Note: MBE/WBE package shall be submitted on a bid due date and time in a separate sealed envelope. These Terms and Conditions shall apply unless otherwise noted in Special Terms and Conditions attached to the solicitation. 1. INVITATION TO BID: a. The Baltimore City Public Schools (City Schools) invites all interested and qualified Bidders to submit sealed bids for the procurement of goods and services in accordance with directions specified in these Instructions to Bidders. b. DEFINITIONS: For the purpose and clarity of this document only, "City Schools" will mean the Baltimore City Public Schools and/or the Baltimore City Board of School Commissioners. "Board" shall mean the Baltimore City Board of School Commissioners. Also, for the purpose and clarity of this document, “Bidder" will mean any reliable and interested broker, contractor, and/or manufacturer that want to bid on this solicitation. LIMITATIONS: This solicitation does not commit the Baltimore City Public Schools to award a contract for this solicitation or to pay for any cost incurred in the preparation of the bid response. The Baltimore City Public Schools reserves the right to accept or reject any and all bid responses received as a result of this solicitation as well as cancel in part or in its entirety this solicitation. SUBMISSION INSTRUCTIONS: Bidders shall submit responses in the order and form listed below to expedite accurate evaluation by City Schools. Bids that do not conform to these instructions may be rejected. a. Each Bidder shall submit one (1) hard copy and one (1) electronic/soft USB flash drive copy in PDF format with bookmarks corresponding to requirements outlined in this solicitation. If these requirements represent a problem to the responding firm(s), please contact Mr. Mike Krupnik at 410-396-8826. Each USB drive shall be identified by professionally made labels with solicitation number and title, company name, address and firm contact person’s telephone, fax and email address. USB drive label shall be attached by the means of a key ring for identification and filing. b. The Bid responses (both hard and soft copies) shall be prominently identified with City Schools Solicitation number, title, submitted date, and the name and address of your Company and prepared in order listed below. The Company name, contact person’s name and title, telephone number, FAX number and e-mail address must appear on each page of the submittal documents included in the Response Package. 1. Copy of State of Maryland Certificate of Good Standing or other filing verifying the Bidder is in Good Standing with the Department of Assessments and Taxation of Maryland. 8. City’s YouthWork Program forms - Baltimore City. 12. The MBE and WBE solicitation requirements shall be returned in a separate package from the bid response. The MBE/WBE package is due at the same date and time as the solicitation. CONTRACT DOCUMENTS: All Bidders responding to this solicitation agree to comply with the City Schools Standard Form of Agreement and AIA General Conditions as modified by City Schools, as well as all terms and conditions, addendums, clarifications, specifications, drawings and other documentation and requirements of the solicitation. These documents shall form the Contract Documents. All work performed under this contract and resulting from this solicitation shall be in accordance with the documents as listed herein, any addenda, and other components of the Contract Documents. The contract resulting from this solicitation is the City Schools Standard Agreement that addresses the project(s). The apparent most responsive and responsible low bidder responding shall after receipt from City Schools of a Notice of Intent to Award letter, return to City Schools the following within ten (10) business days: A. Two (2) original Standard Form of Agreements between City Schools and Contractor fully executed, signed and witnessed (Agreement sample attached with this solicitation). B. Selected contractor(s) shall provide to City Schools annual Payment and Performance Bonds in the total amount of annual contract award stated in the Notice to Proceed. (Payment & Performance Bonds shall be provided as contractors overhead expenses without any additional cost to Baltimore City Public Schools). The cost of purchase
