The views expressed in this report do not necessarily reflect those of the funder. v GLOSSARY Individual interview: research interview consisting of one participant only. ‘Friendship’ pair: a research interview involving two participants, who were known to each other and elected to take part in the study together. ‘Friendship’ group: a group discussion involving three or more participants, who were known to each other and elected to take part in the study together. Young people / youth: whilst the authors recognise that in different contexts these terms can refer to specific age bands, the terms are used interchangeably in this report. Snowballing: a method of recruitment where participants identify people they know who may be interested in taking part in the study. This approach has shown to be effective in reaching ‘hidden’ populations, such as drug users (Faugier & Sargeant, 1997). vi EXECUTIVE SUMMARY OF RESULTS • The use of legal party pills (LPPs), also known as ‘social tonics’, ‘herbal highs’ and legal party drugs, has become a popular activity in New Zealand, in particular amongst young people (Wilkins, Girling, Sweetsur, Huckle, & Huakau, 2006). It is estimated that 50,000 four pill packs are sold every month and that the industry has sales of $24 million dollars per year (Perrott, 2005). These substances contain chemicals such as benzylpiperazine (BZP) and m-trifluoromethylphenylpiperazine (TFMPP). BZP has been shown to have amphetamine-like activity (Campbell, Cline, Evans, Lloyd, & Peck,

To overcome the abuse by these substances, strict legal laws were constituted globally. However, novel and drugs with chemical structures similar to illegal psychoactive drugs substances (with a slight structural change) were manufactured in undercover laboratories to have the same or augmented psychostimulatory effects. Currently, the major classes of designer drugs are piperazines, cathinones, synthetic cannabinoids, synthetic opioids, tryptamines, and phenethylamines. These classes of designer drugs have shown to elicit significant psychostimulatory effect by a different mechanism of action. There are very few reports on the dopaminergic neurotoxicity of piperazine derivatives. However, they have shown to affect various monoaminergic neurotransmission. It has been sold legally in New Zealand in the form of ‘party pills’ since 2000. The legal status of BZP party pills has been debated in New Zealand as the media reported cases of apparent overdoses and adverse reactions leading to hospitalization (Brogden, 2005; Crewdson, 2007; Reiber, 2005; Rankin, 2006). Representatives of the BZP party pill industry publicly defended their product claiming that BZP party pills were reducing substance related harm by reducing illicit substance use (Bowden, 2007b, p.1). They also claimed that banning BZP would result in an increase in use of illicit substances, especially methamphetamine or ‘P’ (Barnett, 2007). The overall aim of this thesis is to test the claims that BZP party pills reduce substance related harm by reducing illicit substance use, and to identify potential outcomes of a BZP party pill ban. In addition, the perceived risks of party pill and other drug use will be examined. In chapter one I review key concepts relating to BZP party pill use: recreational drug use, harm reduction, and risk perception. In chapter two the history and New Zealand context of BZP party pills are reviewed.

    Why have pill testing when most ecstasy deaths are from normal doses of MDMA? Eight central issues for Australian Parliamentarians 1. There are no mysteries about party pill deaths in Australia. Almost all, according to the many Coroner’s reports, are from ecstasy itself 2. Very few party pill deaths in Australia have been from unknown other drugs contained in ecstasy pills, and the only Australian study on ecstasy-related deaths mentions no Coroner-reported deaths from other contaminants or impurities 3. Ecstasy overdose is rare, with most dying from MDMA used at normal recreational levels or in combination with other legal or illegal drugs. Many die because of something akin to an individual allergic reaction to MDMA 4. Pill testing’s false sense of safety will only broaden the pool of MDMA initiates, which will ipso facto lead to many more users fighting for their lives in Australia 5. Pill testing is inadequate compared to ever -evolving substances potentially in party pills 6. Pill testing will not deter the use of party pills 7. European studies on the claimed ‘success’ of pill testing fail to demonstrate or even measure reduced mortality 8. There is likely another agenda behind the pill testing push – the normalisation and legalisation of illicit drugs in Australia Central Issues & Compiled Evidence NOTICE TO DECISION MAKERS This paper has been prepared to inform Australian decision makers on what the medical and scientific literature and studies reveal concerning MDMA-related deaths in Australia. An estimated 20 million doses of these party pills have been sold legally in New Zealand over the past six years. There is a significant gap in the research on the clinical effects of BZP and TFMPP, with only two small clinical trials on BZP published in the 1970s, and no clinical trials on TFMPP. This situation led to the Expert Advisory Committee on Drugs (EACD) recommendation that more information on the effects of BZP and TFMPP should be obtained. We conducted a randomised double blind placebo controlled trial which assessed the effects of BZP/TFMPP alone and in combination with alcohol (six standard units). The doses of BZP/TFMPP (300mg/74mg over a two hour period) were based on those recommended by the party pill industry. The six units of alcohol given was the maximum dose recommended by ALAC to be taken in a session.
