Application Form for Callywith College Bus Pass Summer 2020 APPLICATION FORM FOR CALLYWITH COLLEGE BUS PASS SUMMER 2020 Please read the Terms and Conditions on the back of this application form, before applying for your Bus Pass. Please apply by Friday 6th March 2020 Summer Term £160 Please complete and return all sections of this application form to: - Transport, Callywith College, Old Callywith Road, Bodmin, PL31 2GT and pay by one of the methods below. Payments to be made for Summer Term by Friday 6th March by: 1. Cash – In person at the Temple Reception 2. Cheque – Attach a cheque made payable to ‘Callywith College Trust’, with student name and address on the back, to this form before returning to the address above by post or in person at Temple Reception. 3. Credit/Debit Card – Call 01208 224000 or pay in person at Temple Reception. If your household income is below £31,000 you may be eligible for a reduced rate bus pass (via a bursary). Please download a Bursary Form from www.callywith.ac.uk/transport, call 01208 224460 or email [email protected] and ask for a copy. A bus pass on this scheme offers flexibility of travel at a considerably reduced rate. Plymouth Citybus will provide main route bus services to arrive at the College by 09.05 and to depart after 16.15. Students will need to make their own way to and from home to the nearest bus stop. The Callywith College Bus Pass can also be used on Evenings, Weekends and Holidays in accordance with the conditions below. Callywith College has no control over changes to normal service routes, or loss of routes, this responsibility does not lie with us, but we will make every effort to assist if this happens. Plymouth Citybus will allow all Callywith students in possession of a College Bus Pass to use the service as per Terms and Conditions below. Lost Passes If you lose your Bus Pass please inform the College immediately. The loss must also be reported to the Police at reportmyloss.com for a reference number. A charge of £15.00 will be made for any replacement. Refunds Refunds will only be considered by Callywith College in the event of a student withdrawing from College or changing address. CALLYWITH COLLEGE SEASON TICKET SPECIAL CONDITIONS ACADEMIC YEAR 2019/20 1. The travel pass can be used between the inclusive dates shown. 2. Students will be expected to travel to College on the Callywith timetabled services where such services are operated, as these are provided on behalf of Callywith College as part of a contract. Travel will however be permitted on other Go Cornwall local services where a college service is not available. 3. Notwithstanding condition 2 the holder of a Callywith College Travel pass may use the pass at any other time on Zone 4 Go Cornwall services only. 4. Callywith College Travel pass holders will be permitted to use the pass during the Holiday following the term for which the pass has been used but it will EXPIRE before the new term commences. This facility will also apply during the summer holiday break where an expiry date of 31st August will be applied. 5. The Travel pass must be shown to the driver on every journey. If no pass can be produced at the time of travel then the ordinary Adult single fare must be paid. Altered, defaced or damaged tickets will not be accepted and will be withdrawn. 6. The ticket is NOT transferable and does not give the holder priority over other passengers. It is issued subject to the rules and regulations of the company. 7. Plymouth Citybus will refuse to allow travel to any passenger who is abusing the rules and regulations of the Company. Please Note: • The pass is not valid for induction day. • Your receipt can be used for bus travel on the first day of term as proof of purchase. • The first day of the Summer Term is Tuesday 14th April. • You will collect your pass from College, we do not post bus passes to students. • Plymouth Citybus will not accept out of date passes. • This pass can only be used on Plymouth Citybus contracted and Z4 Go Cornwall services. There were 9 reported crimes for February 2013 1 other burglary, 1 theft of a vehicle, 4 other vehicle crimes (1 criminal damage & 3 thefts from vehicles), 2 drink, drug, miscellaneous crime and 1 criminal damage. PCSO Phillips’s report also advised as follows:- ‘there has been a rise in burglaries and car crime in Lostwithiel. Overnight on Friday 1 March two more vehicles were broken into with one having property stolen.
