Use our detail-specific search functions to browse donors and narrow your list. Once you’ve decided on your favorites, more in-depth information is available. Prior to receiving any vials from ESB-USA, we ask that you fill out any forms related to your purchase.

Our Recipient and Storage Agreement outlines the terms you and ESB-USA agree to in purchasing, storing and/or using donor semen. Every client is asked to complete this form and return it to us before vials are received.

The Clinic Release form provides prior written authorization from the licensed health care professional you are working with. This document applies to purchases from clients who request to have vials delivered to any location in New York State or a location that is not a clinic (i.e. for an at-home insemination). For New York State clients, this form can only be signed by a physician, physician’s assistant, or nurse practitioner. For at-home inseminations, it can be completed by a physician or other licensed health care personnel. Both forms may be submitted via fax or email.

Once you have decided on a donor, you should order the specimens as early as possible. You may order donor sperm directly on our website or over the phone through one of our trained staff members. You can pay by credit card including Visa, MasterCard, Discover, and American Express. Please know the vial type and quantity you wish to order upon the time of purchase. 4. Shipping or Storing Vials

Upon placing your order, you can also note where and when you want your vial(s) to arrive. We ship to any worldwide destination, so call us for details.

In addition, we offer storage options for a long term plan. If you wish to make sure that sperm from your favorite donor is available for later use, you can reserve vials ahead of time and store them at our facility until needed. A storage account ranges from a minimum of 1 month to 5 years.

Occasionally, a client changes their mind about the donor they purchased and have units of sperm placed in storage. This can happen as new donor options become available with desired characteristics or the stored donor’s overall vial supply runs low.

Whatever the reason, we are proud to offer the Vial Exchange & Buy-Back Program to ensure that you have available to you the best donor possible. There is no cost to you for exchanging comparable vials, as ESB-USA wants to make sure you are 100% confident in your donor selection. Buy back vials are purchased by ESB-USA for 75% of the original purchase price.

In order to be eligible for the vial exchange or buy back program, all vials must have been purchased and stored at ESB-USA without ever leaving the premises. Samples that have left our facility are no longer eligible for the exchange or buy back feature.

Vials will only be exchanged for equal or lesser value vials to those currently being stored. Should a request be made for more expensive vials than those in storage, the client will be responsible for the difference.
