Understanding Family, Social and Health Experience Patterns in British Bangladeshi Families: Are People As Diverse As They Seem? Background: British Bangladeshis are one of the most economically deprived communities in Britain, with high morbidity and mortality rates from chronic illness. Access and use of their services is perceived by Primary Health Care Teams (PHCTs) to be difficult, due to communication and cultural barriers. Methods: One-to-one tape- recorded interviews were held in Sylheti, Bengali or English with an age-stratified sample from the community registered with a practice in central Cardiff. The N*DIST package was used to analyse data, with ongoing discussion of emerging themes. The topics explored in these interviews were family structure and decision making within families, meal patterns, health beliefs, experiences of primary care and barriers to engaging with the outside world. Findings: Family structure and social patterns had many similarities with those of the local community, and dietary and health beliefs also followed ‘Western’ concepts. People were anxious to be healthy, but often did not know about core primary care services. The community places value on the opinion and support of primary care professionals. However, a major cross-cutting theme was difficulty in accessing health care (especially for women), and reasons for this are discussed in the paper. With this information, the PHCT can now consider adapting itself to improve access and communication. We suggest that our methodological approach is both relevant and achievable for those working in primary care settings in our increasingly multi-cultural, ethnically mixed communities, and is not purely the province of sociologists or academics (important learning points have been identified and highlighted). Box 1 Background to the Bangladeshi community in Britain Young men from Bangladesh (then East Pakistan) first started settling in Britain in the mid-1950s, mainly from the poor rural district of Sylhet. They were often seamen, who took jobs as kitchen workers in London hotels. The temporary nature of their stay, together with difficulties in competing with indigenous workers who had the benefits of education and language, resulted in poorly paid jobs and a low standard of living. Wives and children joined in the early 1980s, once it was recognised that it would not be practicable to move back to Bangladesh permanently. These families came with no knowledge of English. Basic schooling and integration into mainstream British life has been a tremendous struggle for many. The Bangladeshi community makes up 0.5% of the total population of the UK. There are approximately 8000 people in the Bangladeshi community in Cardiff, mainly employed in the restaurant trade (2001 Census data). The practice used to approach patients for this study had 311 adult patients registered from the Bangladeshi community, from a total list size of 5000 patients.     West European Politics, Vol. 29, No. 4, 736 – 756, September 2006 Muslims, South Asians and the British Mainstream: A National Identity Crisis? RAHSAAN MAXWELL Current popular opinion assumes that Muslims and South Asians in Britain develop anti-mainstream identities because they live in impoverished and segregated ghettos, participate in non-mainstream religions, and politically organise via ethnically and religiously motivated networks. This article uses survey data from the 2003 Home Office Citizenship Survey to challenge each of those points. First, it shows that Muslims and South Asians are almost as likely as whites to identify themselves as British. Second, it argues that discrimination is more important than simple socio-economic difficulties for British identification. In addition, it claims that despite living in ethnically segregated neighbourhoods and retaining ethnic and religious social and political networks, Muslims and South Asians have actively built integrated networks, have trust in mainstream political institutions, and are committed to being a part of the larger British community. Since the late 1980s ethnic minority politics in Britain has shifted from ‘black’1 politics of inclusion and equality to Muslim and South Asian demands for distinct religious and cultural rights (Ansari 2005).2 Some commentators link this rise of Muslim and South Asian politics to their growing economic and social isolation, and use the summer 2001 riots in Oldham, Burnley and Bradford and the recent London terrorist attacks on 7 and 21 July 2005 as examples of the dangers of poor, alienated, religiously and politically radicalised Muslims and South Asians.3 Public discourse claims that Muslims and South Asians are alienated from mainstream Britain because they live in impoverished ethnic ghettos, participate in non-mainstream religions, and politically organise via ethnically and religiously motivated networks (Leiken 2005; Reiff 2005).

This amounts to 0.5% of the total population of the UK and 6.1% of the ethnic minority population (unless otherwise stated, figures are based on data from the 2001 census). Of these, 154,000 people were born in Bangladesh (Kyambi 2005), which equates to 0.27% of the British population (BBC, 2005). The following table shows the growth in the Bangladeshi population since the 1950s. Table 1: Estimates of the Bangladeshi Population in the UK 1951-2001 Year (census) Total Number of Population 1951 2000 1961 6,000 1971 22,000 1981 65,000 1991 163,000 Actual 2001 283,000 (Source: Peach, 2005: 24) Age and Gender Profile The Bangladeshi population is an overwhelmingly young one, with a median age of 18 (compared to 37 for the White population) and almost 40% of the Bangladeshi population is under the age of 16. This was twice the figure for the White population more generally where only 19% were under the age of 16. Most Bangladeshis were within the 16 to 64 age group (ONS, 2002). The age structure of the newer Bangladeshi born immigrant community also followed the general trend with over ninety percent of that population aged under 45 (Kyambi 2005).
