Children entering the first grade must be six years of age on or before September 1 of that school year. Children entering second grade or higher should present the most recent report card or transcript for registration. KINDERGARTEN HEALTH ASSESSMENT In addition to the immunization requirements, a new NC State Law (Senate Bill 293) requires a child who is entering kindergarten to receive a health assessment. It is the responsibility of the parent or guardian to secure the health assessment for the child from a private doctor or the local health department. The health assessment results must be submitted on the KINDERGARTEN HEALTH ASSESSMENT REPORT form to the child’s principal. Forms are available through the health provider or the child’s school “The Kindergarten Health Assessment” is a public health laws which was enacted to ensure that each child entering kindergarten has a healthy beginning and that school personnel are alter to any related needs of the child that may affect his/her school performance.
