EMERGENCY RESPONSE TO COVID-19 in Mixed Migration Context in and Herzegovina

67,500 COVID- 19 screenings in KEY DEVELOPMENTS AND CHALLENGES reception facilities and in out-of-site  Following the rise in COVID-19 cases among PoCs accommodated in TRC Borici, a locations blanket PCR testing was performed for the entire population of this center. Out of 199 tested PoCs, 36 were positive for SARS-CoV-2. All positive cases were asymptomatic or had a mild form of the disease. The recommendations from Public

723 PoCs tested Health Institute of USC implemented after the first reported positive cases were for COVID-19 extended until 27.03.2021. The third floor of TRC Borici was converted into “COVID- 19 zone”, and is currently accommodating 70 PoCs (positive cases together with their direct contacts).

 Additionally, it was recommended to test all service providers who had been inside TRC Borici since the start of the lockdown. PCR testing was performed on 140 service providers, out of whom 12 were positive for SARS-CoV-2. All positive cases were informed, advised to self-isolate and contact local PHC for further support. A short epidemiological assessment was conducted in order to trace potential contacts inside the TRCs, and with the support of Public Health Institute of USC appropriate actions were taken. All positive cases are considered asymptomatic or have a mild form of the disease.  Five confirmed cases of COVID-19 were reported in TRC Miral. 3 positive cases were transferred from provisional camp Lipa, and 2 cases were identified among the TRC Miral general population. All positive PoCs are currently isolated inside symptomatic isolation, together with 5 direct contacts (who were tested negative). They all have a mild form of the disease and are in a stable condition.  Three confirmed cases of COVID-19 were reported in TRC Blazuj, 2 coming from already accommodated population. Upon contact tracing and further epidemiological investigation, appropriate actions were taken in order to prevent the spread of COVID-19 infection inside TRC Blazuj.  Due to the rise in numbers of COVID-19 among PoCs, as well as among service providers, it is urgently needed to improve compliance with infection prevention and control (IPC) measures recommended by health authorities, namely properly wearing a mask, keeping physical distance, practicing hand and respiratory hygiene, as well increasing capacity of preventive and symptomatic isolation and activities of health promotion related to COVID-19 prevention. Furthermore, a blanket screening and testing of all beneficiaries, as well service providers needs to be performed as a part of contingency plan for COVID-19 prevention.  A total of 262 PoCs were discharged from preventive isolation in all TRCs in BiH, and 47 PoCs from symptomatic isolation, on expiry of the isolation and monitoring period. In the same period, 47 PoCs left preventive and 6 PoCs left symptomatic isolation of their own will.

All recorded PoCs received some form of service, and were screened for common communicable diseases, including scabies and COVID-19. Out of all PoCs (1,223) screened, 41% did not report any health concern. If one combines the collected field data and measures the incidence across the attended population (1,223), a total of 48% of PoCs present skin related concerns. A total of 570 PoCs, or 47% of the population, have clear signs of scabies. A total of 18 locations were visited in the area of Bihac, with a total of 877 PoCs medically attended. Health concerns were reported among 65% of the population, with the majority having skin related conditions, namely scabies (53% of all poCs) and musculoskeletal complaints (4% of all PoCs). In Krajinametal, the most common health concern was related to skin infections, with 69% of all PoCs presenting clear signs of scabies. At the old nursing home the most common health concern was also related to skin infections, with 46% presenting clear signs of scabies. A total of 30 locations were visited in the area of Velika Kladusa, with a total of 301 PoCs medically attended. Health concerns were reported among 39% of the

population, with the majority having signs of scabies (30% of all PoCs present at sites in the area). Comparing the areas of Bihac and Velika Kladusa, it is clear that lack of access to basic services in the urban areas of Bihac, due to the current ban on service provision, including healthcare, is affecting the health of PoCs drastically. With more PoCs making their way to USC, and with already overcrowded reception centers, the individual and public health of this population will deteriorate further, to the point of possible crisis and potential negative effects on the health of the general population.

OPERATIONAL PREPAREDNESS AND RESPONSE

 COVID-19 Response in TRCs in BiH: o As the number of COVID -19 cases among PoCs is on the rise, capacity of symptomatic isolation areas has been increased. At the moment, a total of 152 beds in TRCs in USC are available for preventive isolation and a total of 384 beds for POCs with COVID-19 symptoms.

 Coordination with authorities and health actors: o Following the reports of COVID-19 confirmed cases inside TRC Miral, the PHI of USC has shared recommendations in order to prevent further spread of COVID-19 in the reception center: - Quarantine of TRC Miral, which practically means the isolation of all PoCs who are currently accommodated there for a period of 14 days, including a ban on the of beneficiaries, and a ban on admission of new beneficiaries in TRC; - COVID-19 blanket screening of the entire population accommodated in TRC Miral and isolation the suspect cases as per protocol.

- Suspension of all group activities for a period of 14 days as of today. - Daily measurement of body temperature of all PoCs accommodated, for the next 14 days, and separation of febrile ones according to the protocol. - All PoCs to wear a mask, as well maintain physical distance and practice hand hygiene during the next 14 days, especially during close contact activities (dining, using bathroom, etc.) - Daily disinfection of all premises during the next 14 days. - Ventilation of all the premises of the TRC as often as possible (according to the weather conditions) during the next 14 days. - Testing of all PoCs after 7-10 days (if conditions allow). - Testing of all service providers working in TRC Miral after 7-10 days (conditions permitting). - In addition to the obligatory wearing of a protective mask and gloves and maintaining physical distance, all service providers to monitor their health condition on a daily basis and report possible symptoms to health institutions. - Service providers to ensure fix teams and avoid rotation of teams between the TRCs, in order to reduce the possibility of transmitting potential infection to other TRCs in USC.

- Suspension all unnecessary visits of officials and other actors who are not involved in the direct provision of services to PoCs in TRC Miral during the next 14 days, except in case of emergency. o As part of organizational efforts to implement recommended IPC measures, the capacities of the isolation areas in TRC Miral will be temporarily increased in the upcoming days. o On 16 March 2021, the Danish Refugee Council was informed that JUDZKS had been forced to suspend the provision of health care services at TRC Blažuj and Ušivak due to increased pressure on this health care institution and search for legal provisions to continue health care delivery in TRCs in Canton . Daily challenges related to the worsening epidemiological situation and spread of COVID-19 in Canton Sarajevo have been exacerbated by the problem that TRCs Blažuj and Ušivak s have recently been facing: the medical teams of the Public Institution of the Primary Health Center (JUDZKS), which successfully carried out most of health care activities in TRCs with DRC, have not been present in the reception centers since last week. The need for provision of health services in the reception centers is significant. Currently, the delivery of health care in TRCs Blazuj and Usivak relies exclusively on Danish Refugee Council medical staff. DRC has requested an urgent online meeting during the week with relevant health actors since it is necessary to urgently review the current situation and find an adequate solution in order to reduce the spread of COVID-19 among the population of migrants and asylum seekers and reduce possible impacts on public health.

 Measures related to humanitarian staff: o Following the increased numbers of confirmed COVID19 cases among service providers, humanitarian staff working in the reception centers and out of site locations should continuously self-monitor2. In case humanitarian staff have been in contact with persons confirmed positive for COVID-19, they should inform their health provider and follow the suggested measures. Staff exhibiting COVID-19 like symptoms, including fever, should suspend their presence from the reception centers until fully recovered. Additionally, information needs to be shared with DRC medical teams, to ensure contact tracing and mitigating the risk of spread of COVID-19 infection.

Sarajevo Canton authorities announce lockdown over sudden coronavirus spread Sarajevo Canton authorities have announced a partial lockdown for the upcoming weekend, in an effort to stop the sudden surge in coronavirus cases which hit Bosnia and Herzegovina over the last weeks. The decision was confirmed by cantonal prime minister Edin Forto on Monday afternoon.

The lockdown will last from Friday 8 pm through Monday 7 am, when all bars and restaurants, as well as stores except supermarkets, pharmacies and gas stations will be closed. Also, public transportation will be operating as usual and restaurants will be allowed to do deliveries. Explaining the reason for imposing the new restrictions, Forto said the new coronavirus strain appeared to be a lot more aggressive and hospitals are already filled to capacities. For days, the healthcare authorities in Sarajevo Canton have been reporting three-digit coronavirus cases, which prompted the competent authorities to act. The authorities of Bosnia's Federation entity, which Sarajevo Canton is a part of, will decide on Wednesday whether they would introduce new restrictions on its whole territory. DRC for start Magazine – pages 28-31 …The Danish Refugee Council in BiH – with funding from the European Commission Directorate-General for European Civil Protection and Humanitarian Aid Operations (DG ECHO) – focuses on improving access to primary and secondary health care, including psychosocial and mental health services, for refugees, migrants and asylum seekers, as well as on enhancing the overall protective environment, primarily for the most vulnerable persons of concern, including the survivors of SGBV (Sexual and Gender Based Violence). DRC works closely with Institutes of Public Health, Ministries of Health, Clinical Centers, Health Centers and Hospitals, and with the World Health Organization (WHO) in BiH. Each one of the following reception centers in BiH (Sedra, Borići, Miral, Blažuj, Ušivak, Salakovac), has a temporary outpatient infirmary for receiving patients and providing health services for persons in need, as well as for referral to specialist examinations or to follow-up treatment when necessary in hospitals or clinical centers. The medical teams of Primary Healthcare Centers (PHCs) are present in infirmaries on a daily basis. We need to emphasize that infirmaries in reception centers are fully equipped, and the experience and knowledge of medical professionals is at a high level. On average, DRC supported medical teams in infirmaries conduct over 6,000 medical consultations a month, with over 1,200 direct medical interventions per month. COVID-19 testing is included in medical care provided to people on the move as well as treatment within temporary reception centers under medical surveillance or – in case of severe health conditions – treatment in medical institutions, all in accordance with WHO and local health authorities’ recommendations, and with the same level of treatment that is available for the local population. WHO ‘particularly worried’ about Covid situation in and Central The World Health Organization said it is "particularly worried" about the Covid-19 situation in the Balkans and . “We are particularly worried about the epidemiologic situation in the Balkans, as well as many other countries around Central Europe,” Catherine Smallwood, Senior Emergency Officer at WHO Europe, said during a news conference Thursday. FBiH Crisis Staff demands regulating the entry into BiH with a negative PCR test The Crisis Staff of the FBiH Ministry of Health at an emergency session on Tuesday decided to send a document to the BiH Ministry of Security and the BiH Ministry of Civil Affairs again requesting a joint draft decision which would require that all foreign nationals, including nationals from neighboring countries as well as BiH citizens coming from abroad, when entering BiH, must have a negative PCR test not older than 48 hours, counting from the day of entry into the country. They are all accommodated on the separate floor, there are no entrances or exits without complete equipment, the doctor visits them regularly, the temperature is measured regularly, disinfection and ventilation are performed. Vaccines are finally arriving in Bosnia and Herzegovina through COVAX mechanism The long-awaited news from COVAX arrived at the address of the Ministry of Civil Affairs of BiH yesterday. The vaccines paid for by Bosnia and Herzegovina are arriving in BiH. “The arrival of 24,000 doses of diluent is expected on March 23, while 23,400 doses of BioNTech/Pfizer vaccine arrive on March 24. We will inform the public about more details immediately upon receiving more data. The exact time of arrival of the vaccine depends on the flights to Sarajevo,” the Ministry of Civil Affairs of BiH announced.

Vaccines are finally arriving in Bosnia and Herzegovina through COVAX mechanism - FENA.NEWS 22. Bosnia and Herzegovina: Bihac migrant camp quarantined after COVID outbreak The Borici migrant camp situated in Bihac city centre was placed under quarantine until the end of the month following a COVID outbreak, as filmed on Thursday. The camp was closed up after cases appeared ten days ago. The infected migrants are now isolated on the third floor of the facility. Doctors visit them regularly and the necessary epidemiological measures are being taken, according to health authorities. "Unfortunately, the virus has been brought in from two sides, as far as I could interpret. We had one group that has been transferred to the Borici centre and people also had the opportunity to leave for groceries. Maybe they were in contact with their friends," explained Zerina Mulabdic, Director of the Una-Sana Canton Public Health Institute. Hayat TV: Nicola Bay, DRC Country Director about health care for migrants ….The Danish Refugee Council in BiH – with funding from the European Commission Directorate-General for European Civil Protection and Humanitarian Aid Operations (DG ECHO) – focuses on improving access to primary and secondary health care, including psychosocial and mental health services, for refugees, migrants and asylum seekers, as well as on enhancing the overall protective environment, primarily for the most vulnerable persons of concern, including the survivors of SGBV (Sexual and Gender Based Violence). DRC works closely with Institutes of Public Health, Ministries of Health, Clinical Centers, Health Centers and Hospitals, and with the World Health Organization (WHO) in BiH. Each one of the following reception centers in BiH (Sedra, Borići, Miral, Blažuj, Ušivak, Salakovac), has a temporary outpatient infirmary for receiving patients and providing health services for persons in need, as well as for referral to specialist examinations or to follow-up treatment when necessary in hospitals or clinical centers. The medical teams of Primary Healthcare Centers (PHCs) are present in infirmaries on a daily basis. We need to emphasize that infirmaries in reception centers are fully equipped, and the experience and knowledge of medical professionals is at a high level. On average, DRC supported medical teams in infirmaries conduct over 6,000 medical consultations a month, with over 1,200 direct medical interventions per month. COVID-19 testing is included in medical care provided to people on the move as well as treatment within temporary reception centers under medical surveillance or – in case of severe health conditions – treatment in medical institutions, all in accordance with WHO and local health authorities’ recommendations, and with the same level of treatment that is available for the local population….
