Guideline # 18 ORAL HEALTH RATIONALE Dental caries, commonly referred to as “tooth decay” or “cavities,” is the most prevalent chronic health problem of children in California, and the largest single unmet health need afflicting children in the United States. A 2006 statewide oral health needs assessment of California kindergarten and third grade children conducted by the Dental Health Foundation (now called the Center for Oral Health) found that 54 percent of kindergartners and 71 percent of third graders had experienced dental caries, and that 28 percent and 29 percent, respectively, had untreated caries. Dental caries can affect children’s growth, lead to malocclusion, exacerbate certain systemic diseases, and result in significant pain and potentially life-threatening infections. Caries can impact a child’s speech development, learning ability (attention deficit due to pain), school attendance, social development, and self-esteem as well.1 Multiple studies have consistently shown that children with low socioeconomic status (SES) are at increased risk for dental caries.2,3,4 Child Health Disability and Prevention (CHDP) Program children are classified as low socioeconomic status and are likely at high risk for caries. With regular professional dental care and daily homecare, most oral disease is preventable. Almost one-half of the low-income population does not obtain regular dental care at least annually.5 California children covered by Medicaid (Medi-Cal), ages 1-20, rank 41 out of all 50 states and the District of Columbia in receiving any preventive dental service in FY2011.6 Dental examinations, oral prophylaxis, professional topical fluoride applications, and restorative treatment can help maintain oral health. -

Abstract The COVID-19 pandemic resulted in severe limitation and closure of dental practices in many countries. Outside of the acute (peak) phases of the disease, dentistry has begun to be practised again. However, there is emerging evidence that SARS-CoV-2 can be transmitted via airborne routes, carrying implications for dental procedures that produce aerosol. At the time of writing, additional precautions are required when a procedure considered to generate aerosol is undertaken. This paper aims to present evidence-based treatments that remove or reduce the generation of aerosols during the management of carious lesions. It maps aerosol generating procedures (AGPs), where possible, to alternative non-AGPs or low AGPs. This risk reduction approach overcomes the less favourable outcomes associated with temporary solutions or extraction-only approaches. Even if this risk reduction approach for aerosol generation becomes unnecessary in the future, these procedures are not only suitable but desirable for use as part of general dental care post-COVID-19. -

Results: ANOVA (P < 0.05) indicated significant differences in depth and area of demineralized enamel in all the groups. Those teeth treated with fluoride varnish exhibited 50% less demineralization than the control teeth in both the composite and the resin modified glass ionomer cement groups. Conclusion: Fluoride varnishes should be considered for use as a preventive adjunct to reduce enamel demineralization adjacent to orthodontic brackets, particularly in patients who exhibit poor compliance with oral hygiene and home fluoride use. Because the enamel is weaker, teeth can undergo post eruptive breakdown, resulting in missing enamel. Enamel hypoplasia is a quantitative defect of the enamel presenting as pits, grooves, missing enamel or smaller teeth. It can sometimes be difficult to differentiate between the two. In this review paper, we aim to explain the importance of differentiating between the two conditions, and how to manage patients presenting with enamel defects. HOW DOES ENAMEL FORM? Enamel is produced by specialised end-differentiated cells known as ameloblasts.1 The formation of enamel can be separated into initial stages which involve secretion of matrix proteins such as amelogenin, ameloblastin and enamelin, and later stages of mineralization and maturation.1 Tooth enamel is unique due to its high mineral content. It is composed of highly organised, tightly packed hydroxyapatite crystallites that comprise 87% of its volume and 95% of its weight, with the remainder comprising of organic matrix and water.1 This pattern of organisation and mineralisation gives enamel its significant physical properties, making it the hardest tissue in the body.1 Developmental defects of enamel are not uncommon, both in the primary and permanent dentitions.1 Environmental and/or genetic factors that interfere with tooth formation are thought to be responsible for both hypomineralisation and hypoplasia.1,2 If a disturbance occurs during the secretion phase, the enamel defect is called hypoplasia. -

The purpose of the present study was to evaluate and compare the perceptions, knowledge, and clinical experiences of MIH in general dental practitioners (GDPs) and paediatric dentists (PDs) in Spain. Methods: All dentists belonging to the College of Dentists of the Region of Murcia, in the South-East of Spain, were invited to participate in a cross-sectional survey. They were asked to complete a two-part questionnaire including sociodemographic profiles and knowledge, experience, and perceptions of MIH. Data were analysed using Pearson’s chi-square test, Fisher’s exact test and Cramer’s V test. Results: The overall response rate was 18.6% (214/1147). Most respondents were aged 31–40 years (44.86%), with more than 15 years of professional experience (39.72%). They worked mainly in the private sector (84.58%) and were licensed in dentistry (74.30%): 95.45% of PDs had detected an increase in the incidence of MIH in recent years (p < 0.001). Only 23.80% of GDPs claimed to have made a training course on MIH. With respect to the aetiology, chronic medical conditions (p = 0.029) and environmental pollutants (p = 0.008) were the only factors that showed significant between- group differences. -
