SUBJECT: Amendment #4 to T17-RFQ-017 – CTS Centrex Services
Summary: The purpose of this amendment is to correct the Appendix E Price List and Cost Model and amend the procurement.
General Information: Vendors are advised to obtain and thoroughly review the complete, formal RFQ located at: http://watech.wa.gov/procurement-announcements. In this amendment and the amended RFQ released with this amendment, deleted text appears struck through in black font and added text appears underlined in red font.
1) Section 5.15 (M) Abbreviated Dialing is deleted in its entirety and replaced with the following new Section 5.8 (M) Voice Mail:
5.15 (MD) Abbreviated Dialing Vendor must provide abbreviated, 3, 4 or 5 digit, dialing as shown in the Service Availability Workbook - Appendix F) within each Central Office that has over 20 lines. 2) To correct the Appendix E Price List and Cost Model to align with the changes corrections to Appendix F Service Availability Workbook via Amendment 1, Appendix E Price List and Cost Model is deleted in its entirety and replaced with the new Appendix E Price List and Cost Model released with this amendment.
