LES Field Day Volunteers Field Day 2015

LES Field Day will be taking place on Tuesday June 2nd at New Hope-Solebury Lower Elementary School. Parent volunteers are needed to help make our annual field day event successful. Volunteering can be in the am, pm, or both. Responsibilities include running an activity station for Field Day. Due to this important task, we ask that no younger siblings come with you on Field Day. The rain date will be Wednesday June 3rd. In the event of inclement weather, please check the NHS website the day of the event to confirm Field Day is still taking place at www.nhsd.org.

There is a new PA state law (Act 153) that requires that volunteers, chaperones and visitors in our school district to obtain certain clearances. If you have not submitted your clearances to Lisa Driscoll in the main office at the LES, you will not be able to volunteer for Field Day. On the back of this form are the instructions on how to obtain the appropriate clearances. Once you have obtained them, you can send in the volunteer form along with your clearances by April 24th.

After the due date of April 24, you will receive an email to confirm that you are volunteering. Please email Gina Ortiz at [email protected] if you have any questions. ______

Required Clearances for Field Day

There is a new PA state law (Act 153) that requires that volunteers, chaperones and visitors in our school district to obtain certain clearances. These clearances are required to be renewed every three years. This law affects many of our parent volunteers for activities such as lunch/recess coverage, classroom parties, field trip chaperones, and Field Day.

The term clearance means that the individual must obtain the following and give completed forms to the LES Main Office.

1. Report of criminal history from the Pennsylvania State Police (PSP) https://epatch.state.pa.us/

2. Child Abuse History Clearance from the Department of Human Services https://www.compass.state.pa.us/CWIS

3. FBI Fingerprinting https://www.pa.cogentid.com/index_pde.htm

4. In addition to these three items listed above, the school district requires that the Act 24 Arrest/Conviction Report and Certification Form (attached on the back of this form)
