Metro Early College High School One-to-One Laptop Program Computer Liability Release /Payment Form
I understand that the cost to use Metro’s computer equipment for the 2015/2016 school year will be: Chromebook (6-9th grade students) $50.00; MacBook Pro (10-11th grade) and MacBook Air (Seniors) $150.00. Students that qualify for free/reduced lunch under the Federal Free and Reduced Lunch Program qualify for a partial scholarship, reducing the fee to $25 for a Chromebook or $50 for a MacBook Pro or Air. This fee covers the cost of the computer’s operation for the year, including normal manufacturer’s warranty, software and hardware upgrades, and necessary maintenance not caused by user.
My signature below indicates my agreement to assume full liability, including financial responsibility, for the laptop computer, accessories and software assigned to my student as a part of Metro High School’s One-to-One Laptop Program. I understand that the laptop is the property of Metro High School. This agreement does not permit the use or loan of the laptop to others, or the copying of any of its software, or the use of Metro High School internet or email access by others, and I agree to monitor and prevent such usage by others. The laptop is to be returned at the end of the school year. Failure to do so will result in an invoice for the full amount of a replacement computer.
I agree that I am responsible for the full cost of replacing the laptop, any of its accessories, or any of its software in the event of loss or theft, and that I am responsible for costs associated with repair of damage, whether from accident, negligence, or any other cause including unreasonable wear and tear, or damage.
I agree by my signature to assume the responsibility and liability for any inappropriate or illegal acts by my student using this equipment. These inappropriate uses would include, but are not limited to, software licensing violations, software piracy, and inappropriate or illegal activities carried out in part or whole using e-mail, online chat, online forums, or the Internet. Metro Early College High School does not bear any responsibility or liability for any inappropriate or illegal use of this computer. I further agree that my student has no right of privacy in the use of this computer, and that I will assure its immediate return upon request.
Costs to consider: The replacement value: Chromebook - $299.00, MacBook Pro $1200, MacBook Air $1000.

Metro provides this information as a service to you only and not as an endorsement of the company, their products, or services. Should you opt to purchase insurance, the agreement is between you and Worth Avenue; Metro cannot act as an intermediary.
