Attending Physician, San Francisco General Hospital: Attend Trauma/General Surgery Clinic on Monday morning from 9:00 a.m. until 1:00 p.m. Service attending 6-8 weeks per year until December 31, 2011. Rounded from approximately 6:00 a.m. until 9:00 a.m. daily during the weeks assigned as Service attending Evening rounds on the entire service during this time also performing emergency General Surgery and Trauma Surgery on assigned on-call days and on other days as necessary. Attended on elective surgery practice in the field of General Surgery.
Attending: Actively involved interacting with the residents on a daily basis whether or not assigned on Service. Service call 6-8 weeks per year and 2-4 nights/month of 24-hour emergency call.
9/1/2005-12/31/2011: On Wednesday afternoons: Attended the Vascular Surgery clinic from 1:00-4:00 p.m. from September 1, 2005 through December 31, 2011.

TEACHING NARRATIVE Dr. Schecter regularly teaches first-year medical students in the Anatomy Laboratory, third-year medical students in the Surgery 110 Course (serving as a preceptor and lecturer). Fourth-year medical students on their trauma rotation at San Francisco General Hospital as well as interns and residents. Dr. Schecter also directs the Advanced Trauma Life Support Course given to residents one to two times per year. Dr. Schecter was recognized by two Kaiser Teaching Awards (the last in 2006) and in 2008 and 2009 chaired the Kaiser Award Selection Committee. Dr. Schecter is no longer eligible for the Kaiser Award as he has won it twice. Dr. Schecter also serves on the Board of Directors of the Gold-Headed Cane Society and is a member of the Advisory Council of the Haile Debas Academy of Surgical Educators. In addition, Dr. Schecter serves as one of the two faculty advisors for the Department of Surgery Global Health Program and serves on the Advisory Council of the Global Health Scholars Program. Dr. Schecter also gives multiple invited lectures every year within UCSF, within local universities, in local, state, national and international meetings. Dr. Schecter also serves as an instructor in the Combat Trauma Life Support Course in the Medical Core of the Israel Defense Forces, one to three times per year. Dr. Schecter designed and wrote a trauma course which is still used on a regular basis to train medical personnel at the Shaare Zedek Hospital in Jerusalem and has been adapted and used at the Mulago Hospital in Kampala, Uganda as part of the UCSF Department of Surgery Global Health Program.
Additional Academic Activities since January 1, 2004
I was on Sabbatical for 6 months serving as a Visiting Professor of Surgery at the Shaare Zedek Medical Center in Jerusalem, a campus of the Ben Gurion University. I am giving a 15 week course to the hospital staff in Trauma Care. In addition I helped organize and conduct two 2 day Trauma Courses for the Physician Staff. I have given the following lectures since January 1, 2004:
Introduction to Trauma Care Initial Approach to the Injured Patient Airway Management Shock Management of Chest Trauma Management of Abdominal Trauma Management of Head Trauma
I am also studying the Management of Mass Casualty Events. Unfortunately, we have had two suicide bus bombings since I arrived in Jerusalem and I played a major role in the management of the mass casualty event on both occasions. This experience has resulted in the submission of two abstracts dealing with various aspects of organization for mass casualty events. One was submitted to an International Congress on the Medical Aspects of Terrorism and the second was submitted to the American Association for the Surgery of Trauma. I have not yet heard whether the abstracts were accepted for presentation. We are planning a review of the spectrum of injury resulting from Terrorist Bombing incidents to help better plan for the management of these patients.

RESEARCH PROGRAM I am a full-time Clinical Surgeon and have no basic science research program. However, in my capacity as Chief of Surgery at the San Francisco General Hospital, I was intimately involved in the agreement to fund the UCSF Surgical Research Laboratory at the San Francisco General Hospital, the design of this laboratory, and the recruitment of Prof. William Welch, as the Elsbach-Richards Professor of Surgical Research to lead scientific efforts in that laboratory.
My clinical research efforts are in four general areas:
1. The investigation of clinical surgical problems related to injury and infection 2. The investigation of clinical surgical problems related to HIV infection 3. The investigation of the prevalence of exposure to blood-borne infections during surgery and the best methods to reduce the risk of exposure. 4. Ethical issues in surgical practice relating to the treatment of HIV-infected patients and end of life decisions, particularly related to intensive care. 5. Civilian hospital response to mass casualty events.
I have also been intimately involved in a project funded by the Robert Wood Johnson Foundation entitled, “Operation Access.” Operation Access is a 501C-3 Non-Profit Corporation that serves as a coalition of physicians, nurses and hospitals which provides uncompensated, low-risk outpatient surgery to uninsured patients. This program began as a pilot project at the Kaiser, San Francisco Hospital which led to a publication of our early experience in the Archives of Surgery in 1995. Since that time, UCSF/Mt. Zion Hospital and San Francisco General Hospital joined the coalition in San Francisco and we have expanded our operations to Contra Costa, Alameda, San Mateo and Marin Counties. The Robert Wood Johnson Foundation grant ended and we are now a self-sustaining, independently funded organization. This organization has been sponsored by the University of California, San Francisco School of Medicine, the San Francisco Medical Society, the Alameda County Medical Society, and the Northern California Chapter of the American College of Surgeons. Operation Access has been replicated in Delaware and Sacramento and parts of our program have been used in San Diego. I plan to devote a significant portion of my research time to this continuing project to increase access of uninsured patients to surgical care.
MedShare professionals can do a needs assessment at each stage of the program to ensure that the equipment provided is desired, needed and functional.
I am the co-founder of Operation Access and remain active in the organization which is a 501 c3 non profit corporation which facilitates provision of low risk outpatient surgery to uninsured patients who would otherwise be unable to receive surgical care. First established in 1994, the organization has provided over 2000 surgical procedures for uninsured patients in the Bay Area and has served as a template for similar programs in Delaware and Orange County.
In the past two years, I have worked to establish the West Coast branch of MedShare International, a 501C-3 Non-Profit Corporation which acquires, stores and ships medical equipment to hospitals in developing countries. We opened up a 10,000 sq. ft. warehouse in San Leandro in the fall of 2008 and made our first container shipment of equipment to a hospital in Ecuador in January 2009. We anticipate several more container shipments from our warehouse in this calendar year. I serve on the Advisory Council for the Western Region of MedShare International.
