Dear Students: Technicians from U.S. HealthWorks Medical Group. Testing location: 10050 Bubb Road, Suite 3, Cupertino, CA 95014. Check in at the front desk, tell them you have a FitTest appointment, and they will direct you to the testing room. U.S. HealthWorks (Cupertino Medical Group) is located on Bubb Rd just off Hwy 85 and Stevens Creek Blvd., on the west side of the freeway. The clinic entrance and parking spots are located around the back of the building.
HOW OFTEN DO I NEED FITTESTING? Your FitTesting documentation is valid for exactly one calendar year. If you were FitTested in August 2015, you need to be FitTested in August 2016, if you were FitTested in January 2016, you will need to be FitTested in January 2017… and you will not be able to attend clinicals at VMC, O’Connor, Fair Oaks, East Valley, Narvaez, or Regional until you complete this training/testing.
WHAT ARE THE AVAILABLE DATES? Testing will be done in sessions listed below by appointment only. Dates/sessions have been confirmed and phone registration will be available beginning Monday August 15, 2016. All sessions begin promptly and last approximately 2 hours. You will fill out forms when you arrive, so be prompt.

WHERE DO I SIGN UP? Phone registration will be available beginning at 9:00 am on Monday August 15, 2016. Phone (408) 996-8805 between 9:00 am and 5:00 pm Monday – Friday to schedule your session appointment. When you call, identify yourself as a SJSU student and have the following information available for the scheduler: Last name, first name, date of birth, gender, and session requested. COST: The cost to the individual being fit tested is $45.00. Please have cash (exact change) with you when you arrive at the testing site.
DOCUMENTATION: YOU WILL NEED TO COMPLETE FORMS AT THE SITE You will leave with written documentation that you have received this training and fitting. Make a few copies, keep one with your files and give one to your clinical instructor ASAP. It is valid for 1 year. NOTE: *** The facility will charge an $18.00 fee to make copies if you misplace yours! *** YOU NO LONGER NEED TO BRING THE DOCUMENTATION TO THE SCHOOL OF NURSING OFFICE.
APPOINTMENT SIGNUP: Phone registration will be available beginning Monday August 15, 2016. On your appointment day, please wear comfortable clothing and bring a water bottle. Do not eat or drink anything other than water for 30 minutes before your appointment time. Allow 2 HOURS for the appointment. Testing begins when all 30 students registered for the session have arrived and have completed the paperwork. Men: faces should be clean-shaven to achieve a good fit. Please bring a razor with you in case you have to shave to achieve a good fit.
PROBLEMS/QUESTIONS? If you are unable to make your testing date, you will need to de-register and pick another date in advance of your appointment! You will not be able to attend clinical at VMC, Narvaez, O’Connor Hospital, Fair Oaks, Regional or East Valley Public Health until you complete this training/testing. We try to provide appointment options that will work for all students, (including Friday and Saturday afternoon times), so that you can get prepped for your clinical as soon as possible.
If you no-show for an appointment, you will not receive a reminder. · SEMESTER 6 VMC preceptorship students should attend a session.
