Use appropriate hand hygiene at the point of patient contact, e.g. wear gloves if physically handling medication or service user (e.g. administering creams, eye/ear/nose drops)? Check identity of service user? Did staff member make reference to the individuals MAR chart photo or pen picture to aid identification? Correctly identify medication and confirm that MAR chart Instructions match those on MDS &/or meds container label Check any administration details on MAR chart / service user support plan? (E.g. take with food, take 1 hour before food or specific preferences etc). Confirm that the service user was willing and able to receive the medication before opening packets? Extract correct dosage (e.g. correct no. of tablets, correct liquid dosage)? If liquid medication measured, was correct measuring device used, e.g. oral syringe / 5ml pharmacy spoon etc. If “PRN” medication was resident asked if this was required? If dose variable (e.g. 1 or 2) was this offered/given correctly? Offer medication to the service user, with a drink of water if appropriate? Did carer supervise resident until all medication taken?
