VENDOR FAIR REGISTRATION All Strategic Service Partners (SSPs) are required to register for the Vendor Fair with QHR. See the document titled How to Register with QHR for instructions. EXHIBIT COMPANY INFORMATION Freeman is the exclusive exhibit company for the Vendor Fair. You will need to register with Freeman directly to arrange for your booth rental. You can click the attached link, print individual forms and/or order online with Freeman. You may also register by faxing your order forms to Freeman at (469) 621- 5615. Please download the document titled QPA ekit.pdf for the order forms if ordering by fax. If you have any questions about exhibit space, please call Exhibitor Services at (615) 884-5785.

If you have any pop-up displays, please be mindful of the booth dimensions and ensure they do not infringe upon a neighboring booth.

FREEMAN WILL NOT ASSIGN BOOTH NUMBERS. QHR WILL SEND YOU AN E-MAIL APPROXIMATELY TWO WEEKS PRIOR TO THE VENDOR FAIR WITH YOUR BOOTH NUMBER.

You will have access to your booths starting at 1:00 pm. The Vendor Fair starts at 4:00 pm. Due to this limited time, we encourage you to arrange with Freeman to setup any displays that were shipped directly to the hotel. This will ensure your booth is ready to go at 4:00 pm.

Deadline to register with Freeman is Tuesday, May 29, 2012.

All vendors must register with Freeman. If you have not done so, you will not be permitted to exhibit.

Due to space constraints, we request you limit your Vendor Fair participants to two attendees.

Room cost per night: $169 plus applicable taxes

The hotel does not provide airport transportation. Taxi service is approximately $50 one way.

Deadline to register for the hotel is Monday, May 21, 2012.

Strategic Service Partners, as guests of QHR, are invited to attend breakfast on the morning of Wednesday, June 13. Hospital CEOs and CFOs will be in attendance and this will provide our Partners with a good opportunity to follow-up on leads established during the Vendor Fair.

YOUR COMPANY LOGO To ensure maximum visibility for our Strategic Service Partners during the conference, we want to display your company logo throughout the conference. If you are a new SSP account with QPA who has signed within the last 12 months, or, if you are a returning SSP from last year and have a new company logo, please e-mail your company logo in a jpg format to your QPA account manager. The logos will run on a continuous loop on TV monitors located throughout the conference area. We will need your logo no later than Friday, May 11.

PRIZE GIVEAWAY Many of our Strategic Service Partners have enjoyed participating in a prize give-away event at the Vendor Fair. If you would like to participate in this fun activity, please refer to the attached Prize Give- Away form for details. Should you wish to provide a prize to giveaway at your booth, we require the monetary value does not exceed $75. Gift cards (i.e., American Express card) and cash are not permitted. Prize Giveaways are solely at the discretion of the Partner.
