Introduction Why do we collect beneficiary reports? Beneficiary reports are a great tool for us to learn, share and see how TOMS Shoes are reaching children. These reports help us confirm that shoes are being given and understand the impact and effect they are having on children, community members and your program. Beneficiary reports also give us a way to tell the world about your great work!

TOMS may share these profiles with the TOMS community through TOMS communication and marketing channels (including TOMS.com, Eblast, Display, Social Media, Blogs, Campus Clubs, Public Relations, Lookbooks, Giving reports, Retail collateral (Domestic and International), Internal newsletters and education, Giving Trip participant's personal use (Facebook, blog,TOMS Business Cards, etc.).

Beneficiary Profile Part A: Background information  Date of Interview/Profile:  Child’s first name:  Child’s age:  Child’s gender:  Family situation (e.g. orphan, lives with parents, # of siblings, birth order, etc.):  Child’s personal story (challenges faced, likes and dislikes, funny story, goal for future, etc.)  Are these the child’s first pair of TOMS?: Yes No o If no, how many pairs have they received? When did they get their first pair?  Child’s nearest city (region/sub-region):  Community environment/terrain:  Short description of the local economy:  Primary challenges in the child’s community:

Part B: Child’s Thoughts on New Shoes (please use the child’s words to answer)  Have you had shoes before? If so, what kind? What did you like about your previous pair? What did you not like about them? Were you in need of a new pair? What will you use your new shoes for? o If they haven’t had shoes before, what is difficult about not having shoes?  Tell me the story of the day you got your TOMS Shoes.  How did you feel when you were given new shoes?  What is different about wearing these shoes compared to your previous pair?  Do you like these shoes? Why or why not?  How will you take care of your shoes?  Optional: If the child is doing something noteworthy in school or in their community, please provide information on what that is and the child’s thoughts on it. Why do they do it? What is their hope is for the future of this effort? How does this help their community?

If the child has been wearing TOMS for some time already, please also ask these additional questions:  What positive effects have you experienced since you have been wearing your TOMS Shoes?  What is your walk to school like? How is it different with your TOMS Shoes?  What is your favorite thing to do in your TOMS Shoes? Tell me about a fun time that you had in your TOMS Shoes.

Part C: Educator/Parent/Community Leader (please use the person’s words to answer)  Community members’ name, relation to the child, position within the community (if applicable)  How would you describe this child’s personality? Give an example.  What makes you proud of this child?  What do you hope this child will do in their future?  How has GIVING PARTNER’S ORGANIZATION worked with your community to make it better or to provide services that have impacted your community?  How have you seen (or how will) new shoes help children/this child in your community?

Part D: Giving Partner Perspective  Giving Partner staff name, position:  What are the main interventions and support your organization provides in the child’s community?  How does this child directly benefit from those programs?  Why are shoes needed for the children in this community (please be specific)?  How will the shoes given enhance the program and goals that your organization is trying to accomplish in this community? How long has this community been receiving TOMS?  If applicable, how will these shoes help provide access to education for this child?  If applicable, what infections/hazards/health issues will these shoes help protect children in this community from?  What other benefits will new shoes provide to this child?  What was your favorite part of the shoe distribution that this child was part of?

Part E: Photos & Captions: Please provide at least THREE (3) photos of the child you have interviewed. Here are some types of photos that we love to see! Suggestions and Photo Guidelines Below are suggestions and guidelines for completing your beneficiary profile and photo requirement. Profiles and photos following these tips are most likely to be used in TOMS marketing. Please let your Account Manager know if you have any questions! Beneficiary Interviews Choosing a beneficiary to profile We have found that it is helpful to ask the teachers, parents and/or community members for recommendations on children to profile. Ask for children that are outgoing; doing exceptional things at school, at home, or in the community; and are comfortable talking to adults. Please choose a child between the ages of 6 and 16.

Starting a Conversation We’ve learned that starting the conversation with the child while they are with their friends can help open them up (and you can learn more about them). Please ask the deeper questions on the child’s background one-on-one. We want this to be a positive conversation and experience for the child. Please do not feel pressure to ask any question if you feel that it is inappropriate for the child or their situation.

Tone of Voice We know a lot about how your organization gives TOMS from your proposal, but we would love to hear more about your distributions from the voices of the child, community members and staff. Please use a conversational tone in your answers.

Photo Guidelines (for beneficiary reports only) Photos should try and capture the natural smiles, joy or laughter. - Please do not have children/community members make sad or serious faces. - Please ensure that the child is not inappropriately exposed (for example, naked or clothing not covering the body). - Please do not stand above the child and point the camera down. Images should be taken at their level or below. - Please do your best to not capture other brand logos in the shot.

We appreciate ANY additional images that you are able to share with us of children in their TOMS or the shoe distributions. Please note, by sending images to your Account Manager for TOMS use, you give approval for TOMS to use your images through TOMS communication and marketing channels.
