Dear Parent/Guardian,
Your child has qualified for attendance at the environmental science camp offered for sixth grade students at Camp Goddard. The camp is located near Sulphur, Oklahoma and will be operated by members of the Ed Willkie Middle School faculty. We have reserved an area which can accommodate fifty students during the week of March 20-24, 2017. The cost of attending this camp is covered by the district. Your child’s expenses will be limited to the cost of the t-shirt and any supplies that need to be purchased per the list contained in this packet.
A meeting will be held on Wednesday March 1, 2017 at 6:00 P.M. in the Ed Willkie Middle School library for all qualified students and their parents. Information about the camp will be given, and there will be a question and answer period. You will also have the opportunity to meet the faculty members involved.
The following forms are included in this packet:
1. Driving directions to Camp Goddard 2. List of items to bring to Camp Goddard 3. Parent Permission Form 4. Parent Contact Form
The Parent Permission Form and Parent Contact Form must be completed and returned at the parent meeting on Wednesday March 1, 2017. The Parent Permission Form requires your signature and will need to be notarized. For your convenience, a notary will be present by 4:30pm at the parent meeting. You may also come anytime during school hours to have it notarized. Please arrive early to have you forms notarized before the meeting starts at 6:00pm. If you cannot attend the meeting, please arrange to have the form notarized before returning it. If you have any questions, please feel free to contact us.
