HISTORY: History of diagnosis of posttraumatic stress disease and history of blast injury and traumatic brain injury. The patient has now almost completed the course of 40 hyperbaric oxygen treatments and clinically has shown significant improvement.

COMPARISON: Comparison to previous SPECT brain scans of 09/10/2009 and 09/24/2009.

TECHNIQUE: Following injection of 30.5 mCi of Technetium-99m Ceretec, SPECT imaging of the brain was obtained in axial, sagittal, and coronal planes. Images are presented in black and white and in color. Also 3D images are provided.

FINDINGS: There is a patchy type pattern of abnormality in both cerebral cortexes which involves the right parietal and to some degree the right frontal area more than the other areas of the right cortex and on the left it is involving the left frontal and to some degree the left occipital area. The cerebellar areas appear normal. The basilar ganglia areas appear normal. There is questionably a pattern of some enlargement of the ventricular system.

Conclusions  Minimally abnormal SPECT brain scan with still some areas of decreased localization that are shown in a patchy pattern to both cerebral cortexes. The cerebellar areas and basilar ganglia areas are normal. All areas of abnormality show peri-ischemic penumbra. Comparison to previous SPECT brain scan of 09/24/2009 and the first SPECT brain scan of 09/10/2009 shows a significant improvement in the degree of localization in both cerebral cortexes and also the pattern of localization in both cerebral cortexes though there remains areas of abnormality. Further treatment is suggested because of the abnormal areas in the brain but the study does confirm the marked improvement that the patient has shown clinically with the rehabilitation and hyperbaric oxygen therapy.
