STATEMENT OF WORK ELEVATOR/ESCALATOR MAINTENANCE AND REPAIR SERVICES

I. CONTRACT SCOPE/OVERVIEW: The specifications shall include all labor, materials and equipment necessary to perform the elevator/escalator/maintenance described hereafter to provide a complete service and preventive maintenance program. Reference to AGENCY hereafter shall mean the Commonwealth of Pennsylvania, Farm Show & Complex Expo Center.

TERM of CONTRACT: The initial term of contract is for a one (1) year period and shall begin January 1, 2013-December 31, 2013, with one (3 year) renewal option.

ELIGIBILITY REQUIREMENTS: Arrangement for a site visit may be made by contacting Steve McGinley, Pennsylvania Farm Show Complex & Expo Center, 2300 North Cameron Street, Harrisburg, PA 17110-9443, (717)787-5373. A site visit is required.

II. DEFINITIONS OF MAINTENANCE/REPAIR SERVICE REQUIRED: Description of Equipment – Maintenance/Repair Service shall be completed on the following equipment:

III. STAND-BY for FARM SHOW WEEK: January 5 – 12, 2013, approximately 94 hours (maybe less then stated) 10:00 a.m.-10:00 p.m. possible.

SPECIFICATIONS : Furnish elevator and escalator maintenance service, labor, parts, lubrication, materials, equipment, etc. to repair and replace as conditions warrant the elevator equipment and components herein described using trained and experienced employees to keep the specified equipment in a proper, and safe, dependable operating condition for maximum safety and operating efficiency in accordance with the DGS Contract.

MONTHLY MAINTENANCE VISITS: Supplier shall inspect each elevator at least once each month. During monthly maintenance visits, Supplier shall include the following:

A. ADJUSTMENTS: Adjust machinery and equipment as required. Circuit breakers or main line switches, together with fuses for same are excluded. Supplier shall maintain the performance times as outlined in the modernization specification for each unit. These times include door open & close times, door dwell times, car speeds and floor to floor times. In addition to the performance times the Supplier shall maintain a smooth quiet ride for each unit.

B. CLEANING: Remove accumulated dirt, dust, and rubbish from machine rooms, hoist ways, and pits. Furnish cleaning supplies and tools. Cleaning and refinishing of interiors of cars and exteriors of hoist way doors and frames are excluded. Complete hoist way clean downs shall be performed no less than annually, car tops and car door equipment shall be cleaned no less than quarterly and machine rooms and pits monthly as a minimum.

C. LUBRICATION: Lubricate machinery and equipment with materials recommended by the manufacturer of the specific machinery and equipment. Furnish lubricants.

D. REPAIRS: Determine nature and extent of parts and labor required to restore machinery and equipment to satisfactory performance condition.

E. LABOR, PARTS, TOOLS AND SUPPLIES: Provide labor, parts, tools, cleaning supplies, miscellaneous supplies, and lubricants required to complete services. Replacements parts shall be new and genuine parts equal to parts supplied by the O.E.M. or its successor, to maintain system integrity.

1) The Supplier shall service each elevator/escalators as listed in Part II. as deemed necessary by the facility.

2) The Supplier is responsible to supply all staff and equipment necessary to ensure the successful completion of the agreement.

3) Supplier representatives shall be equipped with identification and have available at all times the necessary tools and instruments required to perform elevator/escalator maintenance and repair services.

4) Supplier must own and have available at all times in stock for immediate delivery and installation sufficient supply of emergency replacement and spare parts for the repair of each elevator and escalator.

5) Supplier shall stock and maintain an inventory of tools and/or replacement parts and also have available on short delivery schedules from their own stocking warehouses all compatible replacement components and parts required for the immediate repair of all elevator and escalator equipment under contract.

2 6) Supplier shall have experience and qualifications in the servicing and repairing of the make and type of elevator equipment as specified in Elevators/Escalators to be covered.

7) The services performed will be in accordance with recognized standards and will meet the minimum safety and inspection requirements of the Commonwealth of Pennsylvania, Department of Labor and Industry, Bureau of Inspection.

8) Upon award of the contract, the successful contractor shall submit to the AGENCY sample report forms of lubrication schedules, maintenance schedules, inspection check lists and reports, repair time tickets and reports, and safety tests report forms pertaining to the elevators/escalators listed herein.

9) The Successful Contractor’s service representative shall furnish the AGENCY, as requested, completed reports of each scheduled inspection for each elevator/escalator under contract including inspections made, adjustments, repairs and other work done, faults, defects noted, repairs required, safety hazards noted and completed lubrication schedule of the elevator/escalator equipment as per DGS Contract. . 10) There will be NO allowances for travel time, vehicle/equipment rental, mileage, or travel expenses.

11) The successful contractor’s service mechanic/technician shall check in with the AGENCY’S representative at completion of services visit and submit the reports as required herein.

12) Prior to correcting any serious defect, the Supplier shall notify the Building Maintenance Supervisor who will give final approval for repair.

13) The facility reserves the right to withhold payment for repetitive calls to correct the same deficiency.

V. ROUTINE CALL SERVICE/NON-EMERGENCY SERVICE : The successful contractor shall provide a 24 hour per day, 7 day per week Call/Non-Emergency Service. All such calls shall be responded to within 0-2 hours and shall provide a Routine Non-Emergency Service Mechanic/Technician on-site within 3 hours.

VII. EMERGENCY SERVICE : The successful contractor shall provide a 24-hour per day, 7 day per week Emergency Service. All such calls shall be responded to within 0-1 hours and shall provide an Emergency Service Mechanic/Technician on-site within 2-hours. Emergency is defined as other than normal working hours of the trade. Normal working hours are Monday-Friday, 8:00 a.m.-4:30 p.m. Parts and materials are covered according to the service agreement as stated in Part I.

VII. SHUTDOWN OF EQUIPMENT : All elevator equipment shutdowns or out-of-service requirements requested by the Supplier shall be coordinated and approved by the agency before any elevator equipment shall be removed from service.

VIII. INSPECTION REPORTS : As requested, the Supplier service representative shall furnish the agency completed reports of each scheduled inspection for each elevator under contract, including

3 inspections made, adjustments, repairs and other work done, faults, defects noted, repairs required, safety hazards noted, and completed lubrication schedule of the elevator equipment.

IX. YEARLY INSPECTION SERVICE/SAFETY TEST: The Elevator units shall have a routine yearly inspection done one (1) time per year. As part of these visits, the supplier is responsible to have the State Elevator Inspector present when the required yearly safety inspection is performed.

Work needed due to inspection: If work is required to the elevator as a result of the safety inspection and the work is approved by the Building Maintenance Supervisor , this work will be prescheduled with the supplier to be completed.

The Supplier contractor is responsible for performing all required safety tests and will ensure that the State Elevator Inspector is present during the required safety tests.

X. LIQUIDATED DAMAGES: If the Supplier is called for services, but does not respond, the Supplier shall pay Agency, as liquidated damages, the sum of $75.00 for each failure to respond. If the Supplier fails to arrive on site within the designated time, the Supplier shall pay the AGENCY, as liquidated damages, an additional $75.00 for each failure to arrive at the site. The supplier shall have round-the-clock availability of its Service Center and provide AGENCY with a 24-hour telephone number. If the Supplier fails to arrive within the designated time, the Supplier shall pay AGENCY, as liquidated damages.

Under no circumstances shall the Supplier permit any minor shutdown or breakdown to continue longer than 24 hours after arrival at the Premises without the concurrence of the AGENCY. Repairs or maintenance must be completed within this time frame. If the Supplier fails to complete the repair or maintenance within the designated time, the Supplier shall pay AGENCY, as liquidated damages, the sum of $75.00 for each occurrence of delay.

Agency will not assess liquidated damages where act of God, freight embargoes, strikes, fire, acts of government, or any other cause beyond the cause of the Supplier, prevents or delays performance, provided that the Supplier notifies the AGENCY of such circumstances, and the AGENCY concurs with the information provided.

AGENCY will deduct the amount of any liquidated damages assessed against the Supplier from the monthly payment.

XI. TECHNICAL ASSISTANCE: Supplier shall furnish competent technical assistance on the type equipment installed within twenty-four (24) hours of AGENCY’S request at no additional charge.
