INSTRUCTIONS Proposed Activity and Beneficiary Report - EO-2

List activities from the last budget approved by Grants Administration, using additional pages as necessary. Do not include administration and engineering. If the grant was awarded before December 21, 2002, complete as much information as available for the race categories. New columns have been added for housing activities which now require income level and ethnic data in households, separated by owners and renters.

Direct benefit activities excluding housing

o By Income section . Complete Persons column only, including all LMI income rows. Do not fill out Household columns (Owner-Occupied HH and Renter Occupied HH).

o By Race section . Complete Persons column only. Do not fill out Household columns (Owner-Occupied HH and Renter Occupied HH).

Housing activities

o By Income section . Complete all columns, including Persons, Owner Occupied Households (HH) and Renter Occupied Households, and complete all rows, including all LMI income rows.

o By Race section . Complete Persons column and all Household columns (Owner-Occupied HH and Renter Occupied HH).

All other activities

o By Income section . Complete Total Beneficiaries and Other Categories rows in the Persons column. Do not fill out Household columns.

o By Race section . Complete Persons column only. Do not fill out Household columns.
