New Patients are responsible for providing the following:  Written documentation from the referring doctor with the specific reason why you need to be seen  Any tests and/or blood work from previous doctors  Completed patient information papers enclosed with this mailing  Copies of all Insurance cards  Referral from your doctor (if required by your insurance)

Please send required documents to the office as soon as possible. *Note: All correspondence should be addressed to our South River office*

All paperwork must be returned a week before your scheduled appointment or you may be asked to reschedule.

Confirmation phone calls are made two days prior to appointments. Please call us back to confirm if we have left a message on your answering machine. A verbal confirmation is required at least one day in advance. We reserve the right to cancel any appointment that has not been confirmed. Patients will be responsible for a $50 “No Show” charge.

Please arrive 20 minutes prior to appointment time. Insurance co-pays are due at the time of office visit and are to be paid before being seen by the doctor.

If you need to cancel, please call as soon as possible so we can accommodate other new patients. Please note: there is a $50.00 “No Show” charge if you do not call to cancel.

DIRECTIONS South River Office: From North: New Jersey Turnpike to Exit 9, Route 18 South (East Brunswick – Old Bridge). Approximately 3.5 miles to Racetrack Road (Pathmark on right). Take Racetrack Road jug handle to left, crossing Route 18. Continue to traffic light. At the light, make left onto Old Bridge Turnpike. Turn quick right into parking lot.

From South/Monmouth County: Route 18 North to East Brunswick (Brunswick Square Mall on left, Red Lobster and Denny’s on right). At the McDonald’s turn right for Racetrack Road. Continue right to traffic light. Make left onto Old Bridge Turnpike. Make quick right into parking lot.

Freehold Office: Route 9 to West Main Street, Freehold (Route 537 West). Proceed approximately 2 miles and turn right into 901 (CentraState Medical Center). We are in the Medical Arts Building, Suite 102.
