Attendance (average of Club & Project meetings) – 80% attendance is required for Star Ranking Awards and Market Animal sale as a 4-H Member. Types of Participation –Categories #1-8 (fill in all categories that apply) *Section 3: My 4-H Story – use divider for county judged books (1-2 pages for grades 4 -5, 2-4 pages for grades 6-8, 3-6 pages for grades 9-12) 6 pages maximum. *Section 4: Projects – use divider for county judged books Annual Project Report Forms (for each completed project)–may not exceed 4 pages each Signature of 4-H Member and Project Leader –required for a completed Record Book Complete all areas of project report forms (learning experiences, things made, raised, awards, etc.) Write N/A in areas that don’t apply to project. Expression pages - up to one side of one page per project - optional Jr./Teen Leader Report Form (if applicable) – Required to earn a Jr/Teen Leader pin & patch.
