Dear Parent or Guardian,

Many people are infected in a mumps outbreak in Washington State, and in several other states. Mumps is a contagious disease spread through coughing, sneezing, or other contact with infected persons. Many people infected with mumps are contagious before and after symptoms show up, so they can spread the virus without realizing it.

We want to make sure our students are well protected against mumps. Getting MMR (measles, mumps, rubella) or MMRV (combined measles, mumps, rubella, varicella [or chickenpox]) vaccine provides the best protection against mumps. All students attending school are required to have two doses of MMR vaccine to be in compliance. If an outbreak occurs at our school, students who are not in compliance or have an exemption may be excluded from school.

Our records show:

Please do one of the following:

 Give us your child’s shot record if he or she already got MMR vaccine  Get your child a dose of MMR vaccine as soon as possible  Get a blood test to check for immunity to the disease

After your child gets the vaccine, please bring your child’s shot record to the school office or fill in the dates below and return this form to school. If your child immune to the disease, please ask your healthcare provider for documentation and give this information to the school.
