DIRECTIONS: 1. Please complete this form only if your participation counts have increased since November 2, 2012. If your participation counts have not increased no further action is needed. Type the exact number of test booklets needed (number of additional students to be tested) by subject. DO NOT round up your counts. We already have your initial participation counts from November 2nd. 2. Email the completed form to the Testing Materials Center no later than 5 P.M., January 18, 2013 by doing the following: a. Click on [email protected] which will open a blank email; if not, open an email and type the address. b. Use the “paper clip” icon in the menu to find and attach your completed order form to the email. 2. Make sure that “January Update STAAR EOC ORDER for YOUR CAMPUS NAME” is in the email SUBJECT line.
