I, ______, do hereby agree that I will indemnify and hold harmless Jim Ohmart, Eileen Hatcher, Brian Herod, the Japhet Civic Association, the Japhet Creek Nature Conservancy, Buffalo Bayou Partnership or any person or entity associated therewith, its officers, agents and assigns, and its members, as a result of my voluntary participation in the project sponsored by Japhet Civic Association, Japhet Creek Nature Conservancy and Buffalo Bayou Partnership on November 19, including but not limited to the following activities:

Picking up trash and debris in designated clean-up areas. Using a rake, hoe or shovel to assist clean-up process and planting vegetation. Bagging litter along designated sites and/or collecting piles of trash and litter to be bagged and transporting debris to collection point. Using various cutting and power tools to remove non-native and/or invasive plant growth.

I further represent and certify to said Japhet Civic Association, Japhet Creek Nature Conservancy, Buffalo Bayou Partnership, any officers, agents and assigns that I am physically able to do the various things enumerated above without limitation and that I have no disabilities which might prevent me from doing the same.

I acknowledge this activity may be dangerous due to unknown hazards, chemicals, wastes, animals and wildlife, and other unseen and unknown dangers associated with the creek, bayou and its environs.

In acknowledge that neither Jim Ohmart, Eileen Hatcher, Brian Herod, the Japhet Civic Association, the Japhet Creek Nature Conservancy, Buffalo Bayou Partnership or any person or entity associated therewith have in any way stated or represented to me in any manner that this activity or the bayou is safe.

I certify that either I am eighteen (18) years of age or older, or that my parent or court-appointed guardian has full knowledge of my participation in this event and has given me permission to participate, as evidenced by his/her signature below.
