A letter or phone call from your local Food Stamps office informing you of the date and location of your appointment. Interviews can also be over the phone.
SOCIAL SECURITY NUMBERS Social Security numbers for all household members
PROOF OF IDENTITY Driver’s License, Passport, ID card with your picture on it
PROOF OF RESIDENCY Something showing your physical address (letter, bills, etc.)
PROOF OF UNEARNED INCOME Unemployment, Veterans Benefits, Child Support, Social Security, SSI, Contributions
PROOF OF WAGES Proof of wages for all working members of the household. Bring all stubs form checks received in the last 30 days prior to your interview. Or a statement from each employer verifying gross income: weekly, bi-weekly, monthly.
PROOF OF TERMINATION OF EMPLOYMENT Verification of when anyone in your household who is not working quit, was fired or laid off in the last 30 days
PROOF OF RENT/SHELTER EXPENSES Lease, rent receipt showing landlord’s name, mortgage agreement, property tax, home insurance. If you live with someone, this person may write a statement showing you live in the home with them and the amount of rent you pay
PROOF OF CHILD CARE EXPENSE A written statement or other proof of payment from the person or business providing care for your child/children
PROOF OF MEDICAL EXPENSES Proof of medical expenses for anyone age 60 or older. Or proof of those eligible for or receiving disability benefits
