INSTRUCTIONS

Distribution: Attach this Form KS-ADS-26 to a payment estimate as follows: Original to state office Copy to government representative Copy to contracting officer Copy to contractor

Preparation is made by the government representative at the time of first variation in contract price due to modification or variation for each succeeding pay estimate.

If any change that appeared on a previous estimate (and therefore on a previous Form KS-ADS-26) is affected by a subsequent change in unit price and/or status (25% variation to modification, etc.), the original entry(s) should be shown, then lined out, and the new entry made below. Example: An item shown in the 25% variation is later modiied, and that amount is included in the modification.

Items should be grouped by estimate or modification number and then by site number. Total the items below the last entry.
