APPLICATION INFORMATION AND DIRECTIONS:

This scholarship is available for graduating seniors in the State of Florida and The Bahamas Islands who plan to enroll in accredited public or private post-secondary institutions or training programs. Please fill out application completely  Handwritten application will not be reviewed nor returned, but shredded for confidential reasons  Make sure the application has the correct amount of postage and is postmarked by April 15, 2015  Mail the completed application to: ZETA EDUCATIONAL THESPIAN ASSOCIAION, INC. (ZETA) Attn: 2015 Scholarship Committee P. O. Box 6224 Only applications POSTMARKED by the deadline of APRIL 15, 2015 will be reviewed for funding consideration. THERE WILL BE NO EXCEPTION.

AP PLIC ATION P ACK ET MUS T INCLUDE :

 Official transcript of all high and post high school academic credits  Documentation of a cumulative GPA of 3.0 on an un-weighted scale of 4.0  Documentation of having taken either the SAT OR ACT  Three (3) letters of recommendation from the following categories: School official (counselor, teacher, coach, tutor, administrator, etc.) Church or community representative (clergy, coach, mentor, volunteer, etc.) Member of Zeta Educational Thespian Association, civic, government, fraternal or leadership organizations

ELIGI BILITY: High school or vocational school seniors who:

 Plan to enroll in public or private accredited post-secondary institutions or training programs  Have been actively involved in rendering services to their school, churches, social and civic organizations in their communities  Have a GPA of 3.0 or above on a 4.0 scale un-weighted

 The amount of the scholarship award will not be less than $1,000.00  In case of a tie, two awards will be given  The awards will be presented upon proof of acceptance and enrollment in an accredited public or private post-secondary institution or training program. Student will need to submit requested documents listed above by Septembe r 1 , 2 01 5 Applicant(s) recommended for an award will be notified in writing by May 1, 2015.
