Job Title: Hospital to Home - Administration Assistant Salary: £8.50 per hour Contract Term: Permanent (subject to 3 month probationary period) Hours: Part time 22.5hrs per week (Flexible hours available) Covering 7 days a week between: 7.00am to 11.00pm Mon-Fri 10.00am to 6.00pm Sat-Sun Location: Based at 7 Bank Street, Castleford, WF10 1JD or working from home Responsible to: Hospital to Home Manager

Job Purpose: To provide administrative support to staff delivering transport for patients aged 50 and over, where an additional support need has been identified, who have been discharged from Pinderfields Hospital ensuring patients are safe, secure and comfortable on arrival home.

Duties and Responsibilities:  To receive telephone referrals from Pinderfields Hospital and organise the Transport Support Workers to collect clients.  To gather initial client information and ensure the referral meets the guidelines set.  To ensure that communication with Pinderfields Hospital is timely and clear.  To collate, log and store key data; adhering to data protection guidelines.  To record all information on Charity Log (Age UK WD’s information management system).  To follow up on referrals made or any outstanding actions in relation to a client.  To support the Hospital to Home Manager to complete administrative tasks related to the service day to day operations.  To be responsive to the needs of the service as required.  To ensure professional conduct throughout the service and adhere to Age UK Wakefield District’s policies, procedures and CQC (Care Quality Commission) standards.  To be able to work effectively independently and often in a lone working environment.

Other Responsibilities:

Customer Relations  Positively promote the features and benefits of the Hospital to Home Service and Age UK Wakefield District when interacting with patients, carers, relatives, colleagues and other professionals.  Work with medical staff from Pinderfields Hospital in a professional and responsive manner meeting the needs of patients/service within the agreed framework.  Work within the Hospital to Home schedule with the patient, ensuring full understanding of the service delivery and boundaries.  Ensure the highest standard of patient care and service are achieved throughout the operation, including the correct logging of any safeguarding concerns, RAG assessment, patient health and safety and referral pathways.

Hospital to Home Admin Assistant AM/JH/NB January 2018 Page 1 of 3 Staff Awareness  To attend induction training, correct logging of any concerns regarding RAG assessment, patient/own health and safety, potential safeguarding issues.  Adhere to Age UK Wakefield District guidance and training on CQC regulated activity and support.  Support the Hospital to Home Manager by demonstrating a good understanding of and compliance with all the Charity’s policies, CQC requirements and Hospital to Home procedures.  Positively contribute to the reputation of a Hospital to Home service being organised, pleasant and professional with patients, carers, families, colleagues and other professionals.

Public Relations  Support the Hospital to Home Manager in maintaining good relations with patients, carers, families, colleagues and other professionals.  Report any activity that could negatively impact on relationships with patients, carers, families, colleagues and other professionals.

Administration  Only access relevant information via NHS staff and other involved parties.  Maintain absolute confidentiality with regard to patient and Charity information.  Cover for staff members due to planned and unexpected absences.  Work closely with Hospital to Home Manager to resolve any issues within the service quickly and effectively.  Undertake any other appropriate duties required to realise the full potential of the service.

Personal Duties:  At all times to work in a manner that promotes dignity, respect and independence for older people and the values of Age UK Wakefield District.  To work within Age UK Wakefield District’s Policies and Procedures including Health & Safety, Confidentiality and Equality & Diversity.  To be familiar and act within staff responsibilities in line with Age UK Wakefield District’s Policies and Procedures relating to Abuse and Whistle Blowing.  To engage in regular supervision and annual appraisal with the Line Manager agreeing on suitable objectives and performance standards.  To identify and prioritise personal development needs and agree with Line Manager how these should best be met to ensure commercial objectives are achieved.  To be an active and committed member of Age UK Wakefield District, taking part in staff meetings, training and Organisational activities as deemed appropriate by Line Manager.  At such times as required take on other appropriate responsibilities.
