HOPKINS SCHOOL DISTRICT 270 STUDENT ACTIVITIES

FINANCIAL AID REQUEST

This form is to be completed when a student seeks an alternative payment plan for the activity participation fee.

Every Hopkins School District 270 student is required to meet the financial obligations of a participation fee. Many students will qualify for financial aid if they have applied for and meet the requirements for the Free & Reduced Lunch program.

A copy of the Eligibility letter that you receive from Royal Cuisine, stating you qualify for the free or reduced lunch program, must be turned in with this form.

ELIGIBILITY FOR FREE AND REDUCED LUNCH:  Students who qualify for free lunch will pay a participation fee of $25 per activity.  Students who qualify for reduced lunch will pay a participation fee of 50% per activity.

INSTALLMENT PAYMENTS:  We do have an alternative payment plan that includes installment payments throughout the activity season.  All arrangements need to be made well in advance of the beginning of the season and need to be completed and approved prior to any type of participation. All payment plans must be paid in full by season’s end.  Installments will be set up in four payments. First payment will be due at time of registration and then every 3 weeks thereafter. Payments will be put on the parent portal and you can pay with a credit card or bring in cash/check by the due date
