Before sending this form to the Saskatoon Sports Hall of Fame, please ensure all applicable parts have been completed, and that a suitable Black & White photograph (5x7 inches) is included with the nomination. Photographs must include the name of the photographer on the back. A letter from the photographer must accompany all photographs authorizing the Saskatoon Sports Hall of Fame to use the photograph on the Wall of Fame in the Saskatoon Field House and for publicity purposes as it applies to the Induction Dinner and Ceremony.
