1. What are the DEA schedules or code numbers for the Substances we are using?

Also note that Tramadol considered a Schedule IV controlled substance in the state of Illinois.

3. What type of forms can DAR recommend for tracking individual vials and usage of controlled substances?
The Drug Inventory Form tracks individual vials based on drug type, delivery date, lot numbers and expiration dates. DAR has a system in place to issue a unique number for each vial as it is received from the distributor. Drug Inventory Form

6. Do I qualify for the fee exemption on the federal DEA registration? (Item# 10 on the Federal Application):
Yes, as an employee of a state university, you qualify for the fee exemption. However, you may not transfer the registration to another institution and your supervisor is required to place their signature on Item#10 as assurance of your employment.
7. Do I qualify for the renewal fee exemption on the state DEA registration? Yes. In order to receive the renewal fee exemption, the following information should be on the state DEA registration application.
Block 2 – Check “Government Owned Facility” Block 3 – Fill in “University of Illinois, Research Laboratory of (PI)” Block 6a – Enter PI as the contact person
Those holding individual registrations who want to be fee exempt will need to either call the IDFPR to seek a change in the name or reapply.
8. What special procedures are required to purchase Class II DEA controlled substances?
To purchase Class II controlled substances, it is necessary for the DEA registration holder to provide original copies of a DEA-222 form with the order. This requires that all purchase orders listing Class II controlled substances must be mailed (not faxed) to the supplier. 9. Is there a supplier of double lock storage cabinets for controlled substances?
You can find standard “Narcotic Cabinets” through Health Care Logistics Inc. at http://www.hcl-intl.com/ . They range in price from $135.00 to $700.00. Please be aware that DEA regulations require that the cabinet be secured. Regulatory inspection officers checking drug storage facilities will confirm that cabinets/safes are secured properly.
