ANNOUNCEMENT COVERS THE FOLLOWING WORK AREA(S): Community Based Outpatient Clinic (CBOC) – Telehealth This announcement is for one position located in one of the Community Based Outpatient Clinics (CBOC) that may also include Saginaw.

DUTIES: The Telehealth Case Manager provides leadership and direction in providing Care Coordination Home Telehealth (CCHT) services to patients in rural locations. This person is a self-directed professional who assesses, plans, implements, coordinates, monitors, and evaluates client (i.e. patient, family, and other disciplines) care activities, while ensuring and facilitating the achievement of quality in a cost effective manner. The successful applicant will have communication skills that enhance healthy partnership with newly enrolled Veterans, coworkers, and supervisors, often using phone and email. How you will be evaluated as an applicant: If you are selected for an interview, a Performance Based format will be used to evaluate past performance. All applicants will be notified after final offer is accepted by selected applicant.

QUALIFICATIONS for Nurses can be found in VHA Handbook 5005, Part II, Appendix G6 and may be reviewed in Human Resources Management Service.  [Insert qualification]

 Bachelor of Science in Nursing, preferred.

 Current, full, active, and unrestricted Registered Nurse license, required.

 Two to three years of successful clinical nursing experience with progressive responsibility.  Home care and case management experience preferred.
