$311.00 Course Fees include: Checks payable to CCEMS CCEMS Administration Fee ($100) Background Check CCEMS Course Management Fee Extrication Day National Registry Skills Testing Day EMT Textbook and Online access ($190) Comprehensive FISDAP EMT Test ($21)

Additional Costs during course: (Approximated) Uniform – see policy in Course Catalog/Student Handbook $150.00 Diagnostic equipment (i.e. BP cuff / Stethoscope / etc) $100.00

The following fees will be paid towards end of year as student prepares to become certified. NREMT – Application Fee $ 80.00 Texas Department of State Health Services Application $ 64.00 Texas F.A.S.T. Finger Print (for DSHS licensing) $ 41.00

Requirements for Tomball ISD: Physical exam (cost varies) TB skin test (cost varies) Drug Testing ~$16

Due September 7 th , 2017 – 1st Uniform Drill on Thursday, Sept 7th (Double daily Grade)

Uniform and personal equipment (The following uniform pieces and equipment are purchased on your own by 9/7/2017, separate from Course Fees) Required for program participation: - Cypress Creek EMS white short sleeved shirt with Student Patch (Purchase from Lonestar Uniform Supply, 8430 North Sam Houston Pkwy) - Black work pants, black belt, black work boots- may be purchased at store of choice - Stethoscope (Littmann lightweight recommended, but any quality stethoscope will do) - Watch with second hand - Manual blood pressure cuff (not electric) - Pen light - Trauma shears - Eye protection (safety glasses)

Highly Recommended: HOSA Membership Dues (Required if desiring to compete in EMT Skills or CPR/First Aid Competitive events) HOSA membership dues cover Local, State, National HOSA dues along with chapter t-shirt Dues vary between campuses; please contact your HOSA Advisor at your campus for more info. Honor Cord

Students will earn an honor cord for successful completion of the HST Practicum course. More details regarding this can be found in the clinical contract and can be purchased for $10 at the end of the school year prior to graduation.
