ADJUSTMENT OF STATUS CHECKLIST

[ ] PASSPORT (copy of currently valid and all prior passports are needed), I-94 Arrival/Departure Record (White/Green Entry Document) .

[ ] APPLICANT'S MEDICAL EXAMINATION REPORT & PROOF OF VACCINATION, Form I-693 in sealed envelope. (Please see https://egov.uscis.gov/crisgwi/go? action=offices.type&OfficeLocator.office_type=CIV for a list of qualified MD's. The MD must be on the panel of approved physicians and you should not pay much more than $200 for each exam.)

[ ] COPY OF BIRTH CERTIFICATE of applicant (with certified translation if foreign document) (we will provide translation template if needed)

[ ] BIRTH CERTIFICATE/NATURALIZATION CERTIFICATE/US PASSPORT/GREEN CARD of petitioner (Copy)

[ ] COPY OF MARRIAGE CERTIFICATE of applicant, if applicable (with certified translation if foreign document). (Please also forward a copy of the marriage certificate for the petitioner’s prior marriage.)

[ ] COPY OF DIVORCE CERTIFICATE(S) from any/all previous marriages of petitioner/applicant, if applicable (with certified translation if foreign document)

[ ] COPY OF DEATH CERTIFICATE(S) of any/all previous spouse(s) who died while married to petitioner/applicant, if applicable (with certified translation if foreign document)

[ ] DOCUMENTS TO SHOW MAINTENANCE OF STATUS: Copies of Forms I-797, I-20, employment authorization card, school transcripts or letter showing full time enrollment, etc.

[ ] DOCUMENTS TO SHOW PHYSICAL AND CONTINUOUS PRESENCE IN THE USA: Copies of income tax returns, W-2, dental and medical records, DMV records, CA drivers license or I.D., school records, job verification letters, etc.

[ ] AFFIDAVIT OF SUPPORT (Form I-864), with supporting documents:

a. Last year Income Tax Return and W-2 of sponsor b. Letter from Employer of sponsor (if applicable) c. Proof of Property Ownership of sponsor (if applicable) d. Bank Statements of sponsor (if applicable)

[ ] PHOTOGRAPHS (8) “passport style” for applicant and (4) for spouse petitioner (http://www.uscis.gov/files/article/04_08_02Photo_flyer.pdf)

[ ] SPOUSE DOCUMENTS (from the attached checklist)
