The Department had previously put Consultant Corporate Name on notice, by a ‘Construction Inspection (CI) Issue Inquiry’ letter dated (date), involving (a) CI issue(s) relating to:

In response to the inspection issue(s) mitigation, the Department is now submitting the attached final cost data, including costs associated with the required Change Order(s) and DOT’s administrative costs.
In accordance with the Standard Terms & Conditions of the Consultant Agreement with your firm for the subject project, Standard Article 40- Unacceptable Work and Standard Article 44- Standards and Procedures, the Department requests that Consultant Corporate Name respond to the issue(s) and associated costs that have been submitted at a meeting with members of the Division of Construction Services & Materials, as part of CPM’s ‘Potential E&O Resolution Process’, Step 1 Review.
