Job Description

Post: Wellbeing Navigator – North Bristol

Job Purpose: To provide a supported signposting service to primary care patients in North Bristol.

Working closely with GP practices and other healthcare professionals, Wellbeing Navigators will aim to address issues that may be causing or exacerbating health problems. This is not in itself a support service – the aim is to work with individuals to identify their needs, then refer or signpost to other organisations, services or community groups as appropriate. The Wellbeing Navigator will be responsible for building and maintaining the relationships with these services, community groups and other stakeholders – including practice staff at local GP surgeries.

Responsible to: Project Coordinator

Salary: £20,000 pro rata.

Hours: 22.5 hrs/week. Fixed term contract, initially until January 2018.

Location: The Wellbeing Navigator will work from GP practices and community centres across North Bristol, with additional hot-desking space at The Greenway Centre in Southmead.

Key tasks and responsibilities

1. Provide personalised information, advice and support to primary care patients and signpost or refer (with consent) individuals to appropriate activities, services and support which will help meet their needs, circumstances and preferences.

2. Empower patients to maximise the control they have over their lives through enabling them to assess their own abilities, identify goals, take charge of decisions which affect them and improve their ability to self-care.

3. Provide support to general practices to embed the model into primary care. 4. Develop, build and maintain effective relationships with and knowledge of local statutory and non-statutory services; local GP practices, voluntary and community organisations and public sector partners. 5. Obtain and maintain (or if others have already done this, support them to obtain and maintain) information on the range of local services, activities, clubs, groups, destinations, etc. available (asset mapping) and report on any gaps in service that become apparent through the project. 6. Keep accurate records relating to the interactions that take place as part of the delivery of the service, contributing to the collection of monitoring information and preparation of progress reports. 7. Follow agreed processes and protocols for storing and transferring information about patients and ensure that confidentiality is maintained.

8. Assist the coordinator with the communications and marketing of the project.

9. Build a resilient community self-help infrastructure by helping to develop and nurture an active volunteer network.

10. Assist the coordinator to identify ways in which the project can evolve and be embedded to a greater extent across Bristol.

General Duties

To undertake any other additional tasks as reasonably deemed appropriate.
