16 Clinical Practicum I
Introduction
Welcome to your first clinical placement; an introduction to your future MRT career. It will be an observational experience in combination with a functional experience in which communication within a team, within a department and within the hospital will be accessed. The purpose is to initiate familiarity of the site/department/exam room’s geography and what is where, familiarity with the personnel and who does what, familiarity of the processes and workflow, like how to do things and how do they work, familiarity with some emergency and safety tools and processes. Your requirements are to participate in all scheduled activities through dialogue with the team and/or mentoring technologist to facilitate completion of a weekly assignment, logging of observed cases and self-assessment. Each of these tools will be encased in one booklet called the Portfolio.
Portfolio
The Portfolio is a record of your competence and an ongoing audit of quality of work throughout the entire clinical program and must be completed to meet program requirements. In CPI, the Portfolio resembles a workbook that combines your assignments, case log and self reflection for each day. There will be one Portfolio created for each clinical practicum, each designed differently to reflect the individual course requirements but reflective of your progressive learning, and productive accomplishments. Submission of the CPI Portfolio will be to Sherri Pagnan, Room J117 at Algonquin College. The Portfolio can be submitted anytime after your last assignment has been received by the course professor on blackboard but the deadline is the end of week 15 of Term One (December17th, 2010). The point system assigned to your weekly assignments (read “Assignments” below) and all rules pertaining to a late submission will apply to your Portfolio in order to meet course completion requirements.
Assignments Assignments are reflective of the objective requirements for the course and are structured in a manner conducive to cross-referencing your answers with the mentoring technologist, classroom, texts and web search answers. All assignments are to be submitted within one week from the day of observation. They must be of professional quality, written with appropriate information and submitted within the above mentioned timeline to qualify as completed and obtain a PASS. Consideration may be given under exceptional circumstances for redistributing marks and/or scheduling a make-up evaluation in order to avoid a failing (zero) grade. A point system will be attached to each assignment, with late assignments reflecting attainable points diminished by 50% for the first late day and each consecutive day late, where on the third day the attainable mark will be 0. Ultimately, submission is required to meet course completion. Grades of zero may also be assigned for specific tests, examinations, or assignments for any of the following reasons: unacceptable reason for absence; professor was not informed by student according to established time lines; objectives for missed session cannot be met; confirmed incident of academic dishonesty such as cheating, plagiarism, and/or disclosing
17 the content of written or practical exams;
You are required to follow College policies regarding plagiarism. All reference sources must be cited in the proper format (APA style) or as otherwise noted within a specific course outline. You should carefully review College Directives E43 Plagiarism, for more information on citing verbal submissions.
The Portfolio will contain the assignments for you to work on during your clinical placement but completed assignments must be submitted on Blackboard. Assignment answers will be posted on Blackboard every second Friday, starting October 15th, 2010.
Log Book The Log Book section of your Portfolio is just what it means. It is a chart graphing cases that you have participated (through observation or fully performing a case) in your clinical practicum. The look of the logbook will change with each clinical semester to represent your successive staging. In CPI, the logbook will include self-reflection and self-assessment.
Student Conduct Please refer to your 2010 Algonquin Student Manual and Algonquin Student Clinical Manual for details on Student Conduct. Expectations in Clinical Practicum are equal to those referring to the college, courses and lab policies. You must also abide by the accepting clinical site’s code of conduct. Failure to uphold professional standards of practice, will lead to disciplinary actions.
Attire and Attendance Student Dress Code, Personal Appearance and Clinical Attendance policies will be enforced for all Clinical Practicum, including CPI. Any missed time/days due to extenuating circumstances that have been communicated to the Clinical Coordinator or the course Professor, must be made up during remedial time to complete the course requirements. The Remedial time is set for the Tuesday of week 14 in Term One. The assignment associated with the missed day, will carry over to the remedial time and rules linked to assignment due date, as stated in “Assignments” section, will apply.
Work Day The hours of operation for all sites in CPI will be 0800-1600h. You will meet the Clinical Coordinator of the respective site you are visiting for your day of observation at 8:00am. Refer to the Clinical Site Descriptions for directions to the site and location within the hospital to meet your Clinical Coordinator. Due to equipment maintenance schedules, department efficiency and flow issues, the Clinical Coordinator reserves the right to adjust your designated area of observation at their site while maintaining clinical objective requirements necessary for you to succeed.
Parking Parking fees will vary from site to site and the city’s transit system, OC Transpo, stops at all sites in CPI. All expenses are at your cost.
Confidentiality Please refer to your 2010 Algonquin Student Manual or Algonquin Student Clinical Manual on the definition of confidentiality, the responsibility associated and the ramifications if not compliant. Due to
18 the increased flow to each site for CPI, you will be required to sign a Confidentiality Agreement for each site and one for Algonquin College prior to starting the first day of your CPI.
19 Pledge of Confidentiality
I understand and agree that as an Algonquin Medical Radiation Technology student, I must hold all information, verbal and written, concerning patients and their visitors in strictest confidence. This includes medical, personal, social and psychological information.
I acknowledge that I have read the Medical Radiation Technology’s Student Handbook and the Algonquin College policy regarding Confidentiality of Patient Information and agree to comply fully with this policy during my clinical practicum. Furthermore, I understand that failure to comply with this policy will result in an Evaluation and Progression Committee action being taken up to and including the possible termination of my Medical Radiography Clinical Education course.
I recognize that breach of confidentiality in any way may be cause for my dismissal from the program.

20 CONFIDENTIALITY AGREEMENT
TOH is committed to protecting the privacy, confidentiality and security of all personal information to which it is entrusted in order to carry out its mission.
This agreement must be signed by all third parties who have business with The Ottawa Hospital and require access to patient care areas.
In my affiliation with The Ottawa Hospital (TOH), the Ottawa Health Research Institute (OHRI) and the University of Ottawa Heart Institute (UOHI), I understand and agree to the following, with respect to all confidential and/or personal health information that I have access to or learn:

21 CONFIDENTIALITY AGREEMENT 1. During my association with Children’s Hospital of Eastern Ontario (CHEO), I may have access to information and material relating to patients, medical staff, employees, other individuals, or CHEO, which is of a private and confidential nature. 2. Sensitive information needs to be protected in order to respect the privacy rights of the individual. Confidential information is typically disclosed only to authorized individuals, entities, or processes. 3. At all times, I shall respect the privacy and dignity of patients, employees, and all associated individuals. 4. I shall treat all CHEO administrative, financial, patient, employee and other records as confidential information. I will protect them to ensure full confidentiality. I shall not read records or discuss, divulge, or disclose such information unless there is a legitimate purpose related to my association with CHEO. This obligation does not apply to information in the public domain. 5. I shall ensure that confidential information is not inappropriately accessed, used, or released either directly by me, or by virtue of my signature or security access to premises or systems as per CHEO policies. 6. 22 Clinical Site Descriptions
Children’s Hospital of Eastern Ontario The Children’s Hospital of Eastern Ontario is a pediatric health and research center providing outstanding family-centered patient care, pioneering breakthrough research, and training the health care professionals of tomorrow. Over the past thirty-six years, CHEO has established itself as a world-class centre providing leading-edge treatment, diagnostic and laboratory services for children and youth aged 0 to 18 years and houses the Provincial Centre of Excellence for Child and Youth Mental Health and the Ontario Newborn Screening Program. This site is located at 401 Smyth Rd., in the same complex as the Ottawa Hospital-General Campus. Upon entering through the Main Doors to the hospital, please stop at the information desk for direction to the Diagnostic Imaging Department; you will also notice signage along the way. Once at the main desk of DI, please introduce yourself as a student from the Algonquin MRT Program and ask for the Clinical Coordinator, Tracey Smith.

Civic Campus This campus is located at 1053 Carling Avenue across from the Experimental Farm and east of the Royal Ottawa Hospital. There are several entrances that you will have access to during the daytime hours. Once in the building, find your way to the C elevators and head up to the first floor. Follow the signs to Diagnostic Imaging Main Reception (C1) and introduce yourself as a student from the Algonquin MRT Program and ask for the Clinical Coordinator, Stephanie Foster.
General Campus This campus is located at 501 Smyth Rd. in Ottawa, in close proximity to the Children’s Hospital of Eastern Ontario. Once through the main entrance, head toward the escalators and go up to the second floor. Follow the signs for Module X-Radiology, which also contain an orange logo with a skeleton on it. These signs will take you directly to the Main Reception Desk, which is known as ACC. Please, introduce yourself as a student from the Algonquin MRT Program and ask for the Clinical Coordinator, Charles Blanchette.
Riverside Campus
