VSA Nevada Paint and Sip Rental Agreement Name of host______e-mail ______Phone number______Address ______Event date ______Event Time ______Number of guests______x $30 = ______Amount due Artist selected ______Where did you hear about Paint & Sip ______VSAN provides: Use of the Garden Pavilion for 2.5 hours for the party (host may arrive up to 30 minutes before to set up), a fun and talented teaching artist to guide you and your guests step by step recreating the featured painting. All art supplies (16”x20” canvas, acrylic paints, table easels, and brushes), an apron for each participant for the night, area for food/drink with 6‘ table for food/beverages (more upon request), stemless wine glasses, wine opener, ice, water, and a CD player. Host provides: Paper goods, tablecloth(s) for food/beverage table, appetizers/food and wine/beer or a non-alcohol beverage for guests. Facility Rules - A refrigerator/freezer is available for use during your party. Anything left after your event will be discarded. - User is responsible for setting/cleaning up beverages and food. - NO SMOKING is allowed in the garden pavilion, the porches or grounds of the Lake Mansion. - User is responsible for any injury that might occur during the event to host/guests on the grounds or in the building.

The cost of $350 for up to 12, plus $10 for groups over 12 up to 15 maximum. The amount is due with reservation. Cancellation Policy - Cancellations at least 14 days prior to event – Fee returned minus $25 administration fee - Cancellations 13-7 days prior to event – Fee returned minus $100 administration fee - Cancellations 6-0 days prior to event or no shows - no refund.
