On completion of the Form, please bring it together with the following documents to the Research Office, Room Z404, 4/F, Block Z, The Hong Kong Polytechnic University. (i) a recent colour photograph measuring 3 cm by 3.2 cm (ii) your Hong Kong Identity Card or passport, whichever is appropriate

Your student Identity Card must be returned to the Research Office / the visiting Department/School on the last day of your visit.
