Attachment: PCMH@UNC’18 Program- Practice Process Effective 4/15/2017 Owner: Practice Quality, Innovation and Population Health Services FAQs A. Cohort & Practice Participation

A1. Cohort 1 and 4 practices (UNC, UNC PN, High Point and Caldwell) will submit for NCQA PCMH 2014 Recognition by July 1st of 2017. The practice will maintain their NCQA status until recognition expiration in June of 2020.

A2. Cohort 2 and 3 practices are currently NCQA PCMH Recognized; the practices will continue maintain their NCQA PMCH 2014 Standards until the date of their cohort expiration (May of 2018 and June of 2019 respectively).

A3. The PQI team will assist cohort 1 through 4 practices in maintaining their NCQA PCMH recognition status until cohort expiration, quarterly audits will be performed by the PCMH team.

A4. Action plans and any recommendations for improvement will be made by PQI teams at quarterly rounding’s.

A5. Cohort 5 will be the inaugural group to use the PCMH@UNC’18 program beginning in August of 2017.

B. Tools, Recognition Achievement & Support

B1. The PCMH@UNC’18 Rounding and Audit Tool will be administered up to four times per year. The information collected will be assessed to represent the Practice’s stage in their Patient Centered Medical Home transformation journey of Foundational, Intermediate, or Advanced.

B2. The PCIC Practice Transformation Assessment will be administered three times per year.

B3. Practice Safety Rounding will occur in conjunction with standardized tools.

B5. When a practice meets a determined percentage score on all three of the above components the practice will be deemed a PCMH@UNC’18 Recognized Practice.

B6. Practice Quality, Clinical Innovation, and Population Health Services will continue to provide support and resources, conduct practice audits, and recommend assistance to support the practice achievements of PCMH@UNC’18 Recognition.
