Cardigan Welsh Corgi National Rescue Trust Rescue Adoption Agreement

I (we), hereinafter referred to as "ADOPTER (S)", agree to adopt the following Cardigan Welsh Corgi from the Cardigan Welsh Corgi National Rescue Trust (CWCNRT):

I (we) agree to adopt the described dog from the CWCNRT and further agree to the following:

1. To neuter the dog, if unneutered, within thirty (30) days of the date of this agreement, or by age six (6) months, whichever comes sooner and to provide evidence of neutering, in the form of a letter on the veterinarian's letterhead, to the CWCNRT. 2. To provide proper food, water, shelter and veterinary treatment, including annual required inoculations, and to abide by all humane laws. 3. To keep the dog solely as a pet and return the dog to the Rescue Committee if unable to continue to provide home or required veterinary care, and not to sell, give, or otherwise transfer the dog to anyone other than the CWCNRT Rescue Committee. 4. To provide obedience training if necessary for the proper adjustment of the dog to its new environment and circumstances. 5. To keep the dog confined to its home premises by fenced yard or kennel, but not by means of any tie-out chains or similar devices, and to keep the dog solely as a housedog. 6. To notify CWCNRT of any change of address or phone number, or if the dog should be lost or stolen. 7. To indemnify and hold harmless the CWCNRT and its officers and members from any and all loss, damage or expense arising from the negligence or willful misconduct by the "ADOPTER (S)" or the actions of the dog or failure of the "ADOPTER (S)" to abide by the terms of this agreement.

I (we) understand that, while the CWCNRT Rescue Committee has made reasonable efforts to determine the health and temperament of the dog, it makes no guarantees as to health or temperament.
