Sustainability Planning Worksheet

1. Vision. Articulate what your vision is for your EEE program at the end of the year. How do you hope things will have developed or changed? What would you like to see happen 3-5 years from now?

2. What changes or adaptations might be necessary to continue to support each other in sustaining efforts around implementing EBP for children with ASD?

3. Who else in your program/school/community could benefit from the knowledge and skills you hope to gain this year? How can you think about supporting the development of their knowledge and skill around evidence-based practices for children with ASD? 4. Could some staff who received training and coaching this year serve as coaches to other staff in future years? Who in your program will be responsible for maintaining and expanding the learning from this training project?

5. Threats to sustainability. What are some potential threats or obstacles to efforts to sustain the outcomes of this training grant? How can you plan for/anticipate these?

6. Broad-Based Support. Who else might need to be involved in the discussion about sustainability of the outcomes of this training initiative? What role do they play and why is their participation important? How might you engage those people?

6. Resources. What resources might be needed to assist with sustainability efforts? How might you utilize the NPDC on-line training models in the future? 7. Brainstorm specific sustainability ideas based on your responses above.

Sustainability Plan. Over the course of the school year you will be required to create a written sustainability plan outlining and expanding some of the ideas you have begun generating here. HowardCenter staff can help continue to develop this plan over the school year. Plans must be submitted in writing to Clare McFadden at the end of the school year.
