In order to receive consideration for award, one copy of this “Request for Quotation” properly completed and signed must be returned to, and received by the Department of Purchasing, not later than the specified central time. 3. All prices, items and conditions must be shown. Any prices, terms and conditions not shown, and presented after the specified closing time will not be considered in the bid evaluating process. 4. Purchase orders or contracts resulting from this quotation may not be assigned without written prior approval from the Director of Purchasing. 5. The seller agrees to protect the purchaser from all damage arising out of alleged infringements of patents. 6. Unless otherwise specified, the Director of Purchasing reserves the right to accept or reject all or any part of this quotation. 7. All offered discounts will be considered in determining the low bid, and taken when payment is made. 8. Bids and performance guarantee, when required, will be outlined in the specifications below. 9. All prices quoted are to be less federal excise and state sales taxes. 11. It is hereby agreed that the bidder will, if required by law, comply with the Kansas Act Against Discrimination, K.S.A. 44-1030 et seq.

Items and Specifications KUMC seeks On-Call Electrical work at the KU School of Medicine in Wichita to fulfill the requirements as outlined in the attached Contract Drawings and Specifications. KUMC reserves the right to negotiate with selected finalists should it be deemed necessary.

KUMC, Request for Quotation No. 683F18-23 Page 1 of 7 GENERAL TERMS AND CONDITIONS

1. It is the intent of KUMC to permit competitive bidding. It is the bidder’s responsibility to advise KUMC Purchasing, in writing no later than seven (7) days prior to the closing date, if any specification or requirement limits bidding to a single source. 2. Unless otherwise specified, KUMC reserves the right to accept or reject all or any part of a quotation, and to waive technicalities. 3. Except as otherwise indicated, the merchandise quoted shall be in new condition. 4. KUMC reserves the right to award purchases by item, by group, or by lot, whichever is deemed to be in its best interest. 5. In the event of a tie, the award shall be made to a Kansas bidder. 6. Unit price shall prevail in the event of extension errors. 7. When brand names or trade names and model numbers are used in the bid request, it is for quality, style and features. Bids on equivalent items of substantially the same quality, style and features are invited, unless otherwise indicated. However, to receive consideration, such equivalent bids must be accompanied by sufficient literature and/or specifications to clearly identify the items and allow for functional evaluation. If a bid indicates that an item quoted is functionally equivalent and is purchased and found to not be comparable, KUMC reserves the right to return the item at the bidder’s expense, and the bidder will be charged back for any difference in cost for the specified item. 8. If any portion of this bid is provided by a vendor other than the bidder, the bidder remains the prime contractor responsible for fulfilling all requirements of this bid. 9. Samples of items, when required, shall be furnished at no expense to KUMC, and, if not destroyed in the evaluation process, will be returned at the bidder’s expense if requested. 10. Partial payments can only be made with the prior approval of the Project Coordinator. 11. Any conviction for a criminal or civil offense that indicates a lack of business integrity or honesty must be disclosed. This includes 1) conviction of a criminal offense incidental to obtaining or trying to obtain a public or private contract or subcontract or in the performance of such contract or subcontract; 2) conviction under state or federal statutes of embezzlement, theft, forgery, bribery, falsification or destruction of documents, receiving stolen property; 3) conviction under state or federal antitrust statutes; and 4) any other offense to be serious and compelling as to affect responsibility as a state contractor. Failure to disclose an offense may result in disqualification of the bid or termination of the contract. 12. Payment Payment Terms are 2%/15 Net 60 days. If the 60th calendar day noted above falls on a Saturday, Sunday, or legal holiday, the following workday will become the required payment date.

NOTE: Pre-Bid Conference - A mandatory pre-bid conference will be held at 11:00 AM, on January 30, 2018 at the following location:

We are located just off the 8th 9th Street exit off Interstate 135. The University is on the east side of the Interstate. Page 3 of 7 Parking is available in the visitor parking by the flag poles. The meeting will be held in the Sunflower Room on the 2nd floor. When you enter the building by the flag poles, there will be a set of stairs to your right. Go up the stairs and continue to walk straight ahead on the balcony. The Sunflower Room is right as you turn the corner on your left. Attendance is required at the pre-proposal conference. Failure to attend the pre-bid conference will result in rejection of your bid. Impromptu questions may be permitted, and spontaneous unofficial answers provided, however bidders should understand that the only official answer or position of KUMC will be presented in writing.

Contract Proposal

The University of Kansas School of Medicine-Wichita (UKSOM-W) is requesting proposals to provide On-Call Services for Electrical services. The University will rely on the successful vendor’s expertise in the industry to provide these services throughout the University.

Term of the Contract

The University of Kansas School of Medicine-Wichita is requesting a (1) one-year contract with (4) four, one- year renewals. Electrical service projects will be initiated with the vendor, based on priority and available funding.

UKSOM-W Objectives

To have an on-call contractor in the specified Trade that will be available at short notice to perform Electrical work outlined in the Scope of Work below on an as needed basis throughout the University of Kansas School of Medicine-Wichita. Job sizes range from work order sized projects to limited renovations of various Departments on the campus. The Contractor must have the ability and willingness to work with: in-house forces and our currently contracted State on-call architects, engineers, construction forces, and sub-contractors.

Scope of Work

The following are types of services that may be required under this contract.

1) Electrical Distribution System Installation, modification, replacement/removal including but not limited to, 13.8 kV AC to low voltage DC. 2) Troubleshoot electrical circuits and devices as requested. 3) Wiring for fire alarm, DDC controls, security and access control systems. 4) Contractor shall provide all required tools, equipment, measuring instruments and other apparatus. 5) Testing of new and existing Electrical Distribution Systems, e.g. periodic testing required by applicable codes. 6) Provide itemized bids as requested in a timely manner. 7) Provide itemized invoices for payment processing. Must also include copies of original invoices for materials, rentals, and subcontractors included in your invoice. 8) All personnel to have Identifying Means of Skill Levels from a recognized training facility when requested by UKSOM-W, to include training for 13.8kV terminations, Hipot Testing, Arc Flash etc. 9) Most work will be performed during day time hours however after hour, weekend, and holiday work will occur at the convenience of UKSOM-W. 10) Response time for after hour call in to be within 1 hour of notification to have at least one person on site. Page 4 of 7 11) Shutdowns will be scheduled two weeks in advance for planned activity. 12) Coordinate with other UKSOM-W vendors/contractors such as Honeywell, Icon Structures, etc. 13) Inspections will be as required by applicable codes, insurance companies and others. 14) Electrical calculations by a licensed professional when specified for the project, shop drawing submittals and record documents as specified by project documents. All submittals are subject to the review and approval of “Authorities Having Jurisdiction” (AHJ), project engineer and/or UKSOM-W.

Qualifications and Criteria for Award:

The award will be made to the most qualified and responsive bidder whose bid meets the requirements and criteria set forth in this proposal, including the criteria below:

1) Licensed contractor in specified trade in Kansas. 2) Contractor shall be able acquire a bond when applicable. 3) The contractor shall have experience in specified trade for a minimum of 10 years. 4) Must have the ability to service projects in a timely manner. 5) Business must be located within a 40 mile radius of UKSOM-W for a maximum 2 hour response time. 6) List what stamps/certifications the company’s employees carry. 7) Must have a single point of contact and ability to devote same set of workers to projects. 8) It is preferable to have in-house capability to accomplish conventional general construction scopes of work items. 9) Electrical Contractor must be able to staff the projects accordingly for the size of each job and keep the same staff the entire project. 10) List Labor Rates and how they are structured. 11) Previous experience at UKSOM-W is preferable. 12) Price 13) Must be able to accomplish work in and around occupied areas and provide any and all means of separation of construction activities from occupied areas.

Code Compliance:

All work under this contract shall be compliant with local and adopted State Code requirements. All work is subject to inspection by the architect/engineer and the AHJ.

LED Lighting Projects

Page 5 of 7 The project consists of removing and disposal of existing light troffers and replacing with new 2 x 2 LED fixtures. This is not a 1 for 1 replacement.  All troffers in rooms listed will be removed.  Replacement LED fixtures are indicated on the accompanying layout. All existing troffer whips will be removed and new whips will be installed with the LED fixtures. Some rooms will require dimmer switch(s) in the rooms and are marked on the layout. The dimmer switches and appropriate wiring to be installed by electrical contractor. All other rooms will use existing wall switches and wiring minus the whips. Where appropriate the 2x2 grid tees will be supplied by KUMC-W for you to install to secure the light fixture. *Projects 1 & 2 will need to be worked on after 5 PM or on the weekends. Materials provided by KUMC: 2’ tees for suspended ceilings 2 X 2 LED Light Fixtures Bid on each project separately. We request you bid on each project separately and not combine them to a single project. It is anticipated that most of the work to be performed by Contractor’s employee(s) will be on the University of Kansas School of Medicine-Wichita campus. The University of Kansas School of Medicine-Wichita will incur no expenses other than the flat hourly rate stated herein.
